Dear Mr. Hong Lei:
On behalf of REH Company, Broadbent & Associates, Inc. (Broadbent) is pleased to provide this Work Plan
Amendment associated with recent monitoring well installation activities conducted at the Basin Market
#105 located at 5380 South 900 East, Murray, Salt Lake County, Utah (Site or Basin Market #105).
Broadbent had to change drillers during this investigation process as the original drilling company was not
responding to calls for scheduling. PacWest provided an acceptable quote and schedule to facilitate
additional monitoring well installation. Unfortunately, additional waste disposal was accrued due to use
of a hydrovac rig instead of hand augering. The original estimate included disposal of one (1) drum at
approximately $250.00/drum. PacWest had to remove waste from the site generated during hydrovac as
well as two drums with soil cuttings. PacWest costs for waste disposal are estimated at $2,486.00.
Accordingly, please let us know if this waste disposal change is acceptable.
