However, the Division recognizes its information may be incomplete. This letter seeks additional information regarding your water system so that the Division can make a final determination as to its legal status. To that end, attached please find a blank questionnaire, Form for New Public Water Systems, seeking information related to your system. Safe drinking water is essential to public health and therefore important we have accurate information about your system to determine what, if any, of the Safe Drinking Water Act requirements apply. If you do not respond to this information request for within 30 days of receipt, the Division may need to take further action to collect the information independently. This additional information gathering may cause a fee for service billed to you. The Division’s preference is to make determinations after reviewing the information submitted through the attached questionnaire.
