To Whom It May Concern,
The following serves as a final report detailing environmental cleanup and remediation activities
for the incident that occurred at the above referenced location on January I 0 , 2023 . Please see the
site map attached in Appendix 1.
Incident Background:
On January 10 , 2022 , at 1200 ET, approximately 30 gallons of diesel fuel released from a FedEx
Ground receptor containment area due to an unknown cause. The discharged liquids impacted the
onsite retention pond and the soil banks of the pond .
Emergency Response:
On January I 0, 2023, at 1559 ET , FedEx Ground retained Emergency Response and Training
Solutions (ERTS) to manage and oversee environmental operations. FedEx Ground dispatched
Clean Harbors of Tooele, UT to conduct emergency response actions . ERTS later di s patched
GrayMar Environmental Services of Tooele , UT to complete remedial actions .

Regulatory Notifications:
On January 10 , 2023 , at 2345 ET, ERTS notified the Utah Department of Environmental Quality
of the release. Dan Riddle received the call and issued reference number 16079 .
Corrective Action:
On January 10 , 2022 , Clean Harbors personnel arrived onsite and observed a 40ft-by-50ft area of
the retention pond that was impacted as well as the soil banks surrounding the pond. Clean
Harbors personnel skimmed the top of the pond and offloaded approximately 3,000 gallons of
impacted water into a frac tank onsite . Clean Harbors personnel deployed absorbent boom to
secure the site for remediation at a later date . The waste generated was left onsite.
On January 13 , 2023 , Clean Harbors personnel returned to the site to continue pond pumping
operations . Clean Harbors personnel collected an additional 3,500 gallons of diesel impact liquid
and offloaded into a frac tank . Absorbent boom was deployed at the outfall and throughout
heavily impacted locations of the impacted area. Clean Harbors generated four drums of solid
waste which were left onsite.
On January 23 , 2023 , Clean Harbors returned to the site to offload impacted liquids in the frac
tank into a vacuum truck . 3,000 gallons of liquids were transported off site for disposal.
On January 25 , 2023 , Clean Harbors personnel returned to the site to complete a site assessment
to determine remediation requirements and stated that heavy precipitation had caused the
previously drained area to refill with ice and snow . No waste was generated in this response .
On February 06 , 2023 , Clean Harbors returned to the site to offload impacted liquids in the frac
tank into a vacuum truck . 2 ,500 gallons of liquids were transported off site for disposal.
On February 20 , 2023 , GrayMar Environmental Services personnel arrive onsite to begin
remediation of the impacted soil and pond area. GrayMar Environmental Services personnel
created a dike around the impacted area for excavation and then recovered approximately 13,000
gallons of liquid in the bermed area using a vacuum truck. The liquids collected were offloaded
into a frac tank onsite.
On February 21 , 2023 , GrayMar Environmental Services personnel returned to the site to
continue remediation actions. GrayMar Environmental Services personnel stated that overnight
the bermed area refilled with water. Gray Mar Environmental Services personnel pumped an
additional 2 ,000 gallons ofliquids out of the berm area. GrayMar Environmental Services
personnel excavated a 20ft-by-30ft area of soil recovering approximately 60 tons / 80 yards of
impacted soil that was placed into four roll-off boxes . Three confirmation samples were collected
and later ORO analyzed yielding sufficient results . The Analytical Report outlining sample
results is provided in Appendix 2 . The excavated area was then backfilled . GrayMar
Environmental Services personnel deployed absorbent boom at the outfall as a precautionary
measure .
On February 22 , 2023 , Clean Harbors returned to the site to complete impacted liquid transfer
from their frac tank to a vacuum truck for transport for disposal. Clean Harbors used a portable
heater to thaw the frozen liquid in the frac tank and then personnel washed out the tank with a

pressure washer. Approximately 1,000 gallons of diesel impacted water was transported offsite
for disposal. Photos provided by Clean Harbors from all their responses are attached in Appendix
3 .
On March 01 , 2023 , GrayMar Environmental Services personnel returned to the site to transport
approximately 4 ,000 gallons ofliquid offsite for disposal.
On March 02 , 2023 , GrayMar Environmental Services personnel returned to the site to transport
two roll-off boxes of solid waste and approximately 4,500 gallons of liquid waste in a vacuum
truck for disposal. The two roll-off boxes remaining onsite were later removed from the
aforementioned location and taken to their facility in Tooele, UT to be transport to the disposal
facility at a later date.
On March 03 , 2023 , GrayMar Environmental Services personnel returned to the site to complete
liquid transfer to a vacuum truck and conduct boom maintenance. GrayMar Environmental
Serv ices personnel transferred the remaining 1,000 gallons of liquid from the frac tank to the
vacuum truck and then recovered an additional 3,500 gallons of liquid from the pond 's inlet. The
v acuum truck containing the impacted liquid was transported offsite for disposal. GrayMar
Environmental Services personnel returned to the site after offloading the vacuum truck and
cleaned the frac tank and recovered the rinse water and additional water from the inlet. GrayMar
Environmental Services personnel secured the inlet area with absorbent boom as a precautionary .
An additional 2 ,500 gallons of liquid was transported offsite for disposal.
On March I 0, 2023 , GrayMar Environmental Services personnel returned to the site to complete
boom maintenance. GrayMar Environmental Services personnel used a vacuum truck to collect
approximately 5,000 gallons of liquid from the pond and offloaded into a frac tank . The impacted
absorbent boom was recovered and replaced with clean absorbent boom.
On March 13 , 2023 , GrayMar Environmental Services personnel returned to the site to flush the
drain leading to the outfall of the pond and conduct boom maintenance. A pressure washer and jet
rodder were used to flush the impacted drains, sumps, and nearby fuel island . GrayMar
Environmental Services recovered approximately 3 ,000 gallons of rinse water that was
transferred to the frac tank .
On March 21, 2023, GrayMar Environmental Services personnel returned to the site for boom
maintenance and liquid transport disposal. GrayMar Environmental Services personnel inspected
the absorbent boom and left it in place . Approximately 3,500 gallons of liquids were transported
off site for disposal.
On March 22 , 2023 , GrayMar Environmental Services personnel returned to the site for boom
maintenance and liquid transport disposal. Approximately 2 ,800 gallons of liquid was transferred
from the frac tank to the vacuum truck for transport to disposal and then the frac tank was cleaned
out and the rinse water was recovered into the vacuum truck. GrayMar Environmental Services
personnel collected all the spent absorbent boom and deployed additional boom at the outfall of
the pond .
On March 31, 2023 , GrayMar Environmental Services personnel returned to the site for boom
maintenance . GrayMar Environmental Services personnel stated that no sheen was visually
present to the pond or outfall ; however, they stated that light impact to the boom was observed.

GrayMar Environmental Services personnel collected the spent boom and replaced it as a
precautionary measure. All absorbent boom recovered was containerized and left onsite for
FedEx Ground to process for disposal.
On April 10 , 2023, GrayMar Environmental Services personnel returned to the site for boom
maintenance . The absorbent boom was collected after GrayMar Environmental Services
personnel inspected the pond and outfall confirming no impact was observed to the boom . The
absorbent boom that was collected was consolidated into the drums from the previous response
that was left onsite at the FedEx Ground terminal. Photos provided by GrayMar Environmental
Services from all their responses are attached in Appendix 4.
Waste Disposal:
On January 23 , 2023 , approximately 3,000 gallons of non-hazardous diesel , water , absorbent was
transported to Clean Harbors located in Grantsville , UT for disposal.
On February 06 , 2023 , approximately 2,500 gallons of non-hazardous diesel , water, absorbent
was transported to Clean Harbors located in Grantsville, UT for disposal.
On February 22 , 2023 , approximately 1,000 gallons of non-hazardous diesel , water, absorbent
was transported to Clean Harbors located in Grantsville, UT for disposal. Please see the Waste
Disposal Documentation provided by Clean Harbors attached in Appendix 5.
On March 02 , 2023, approximately 4,000 gallons of non-hazardous waste liquid (water) and two
roll-off boxes containing approximately 19.87 tons of non-regulated material were transported to
Wasatch Regional Landfill located in Skull Valley, UT for disposal.
On March 03, 2023 , approximately 7,000 gallons of non-hazardous waste liquid (water) and one
roll-off box containing approximately 20 yards of non-regulated material were transported to
Wasatch Regional Landfill located in Skull Valley, UT for disposal.
On March 08 , 2023 , one vacuum truck containing approximately 3,500 gallons of non-hazardous
waste liquid (water) and one roll-off box containing approximately 11.33 tons of non-regulated
material were transported to Wasatch Regional Landfill located in Skull Valley, UT for disposal.
On March 21, 2023, one vacuum truck containing approximately 4,168 gallons of non-hazardous
waste liquid (water) was transported to Wasatch Regional Landfill located in Skull Valley, UT
for disposal.
On March 22 , 2023 , approximately 3,188 gallons of non-hazardous waste liquid (water) was
transported to Wasatch Regional Landfill located in Skull Valley, UT for disposal.
All drummed waste as noted throughout this report was left onsite for FedEx Ground to process
for disposal. For further information on disposal of this waste , please contact FedEx Ground.

The release has been completely remediated and no evidence was found that suggests any
significant environmental impact remains on the site . ERTS suggests that this project be closed,
and no further remedial action be taken.
