Apologies for my oversight regarding Utah Admin Code R315-263-33, an updated report after 15 days.
The follow-up for the spill occurring January 21, 2025, follows:
(a) The person's name, address, and telephone number;
Chalmer Bitsoi, 218 N Auburn Ave, Farmington, NM 87401, (505) 574-1355
(b) Date, time, location, and nature of the incident;
January 21, 2025, Montezuma Creek, Utah, Leak due to External Corrosion
(c) Name and quantity of material(s) involved;
4 barrels of Crude Oil
(d) The extent of injuries, if any;
No Injuries
(e) An assessment of actual or potential hazards to human health or the environment, where this is
applicable; and
Spill occurred in remote area, 3-5 miles from nearest residence. No impact to groundwater, frozen
ground. Oil recovered and properly disposed. Corroded pipe repaired and place back in service.
(f) The estimated quantity and disposition of recovered material that resulted from the incident.
4-barrels of crude oil recovered with vacuum trucks and reinjected into pipeline at Montezuma Creek
Utah. Approximately 16 cubic yards of oily dirt transferred to San Juan County Land farm for final
disposition.

Hello Chalmer Bitsoi,
I am looking for information on the Crude Oil release in San Juan County, due to a unauthorized discharge
event on January 21, 2025. Please submit a written 15-day report with the information seen below, as
outlined in Utah Admin Code R315-263-33. This report is required to close out the spill incident number
19478 (attached). Let me know if you have any questions.
(a) The person's name, address, and telephone number;
(b) Date, time, location, and nature of the incident;
(c) Name and quantity of material(s) involved;
(d) The extent of injuries, if any;
(e) An assessment of actual or potential hazards to human health or the environment, where this is
applicable; and
