Box 52 Wallsburg, Utah 84082 Subject:Operating Permit, Lower Tank RPL-1 (ST003), new well pump for Town Well (WS002), and water line replacements for UTAH26009 Distribution System (DS001); Wallsburg Town Water System, System #26009, File #13594, SRF #3F1889 Dear Troy Thompson:The Division of Drinking Water (the Division) received a request for an Operating Permit for Lower Tank RPL-1 (ST003), new well pump for Town Well (WS002), and water line replacements for UTAH26009 Distribution System (DS001) from your consultant, Caleb Dotten, P.E., of Sunrise Engineering on January 28, 2025. The Division issued Plan Approval for this project on February 13, 2024. Division staff, Dani Zebelean and Kjori Shelley, conducted an onsite final inspection of this projecton February 19, 2025. Our understanding of the project was the construction of a new 500,000 gallon concrete storage tank Lower Tank RPL-1 (ST003) and abandonment of the existing Lower Tank (ST002), replacement of a 200 gallon per minute (gpm) well pump at Town Well (WS002) which will pump water to the new storage tank, and approximately 15,700 linear feet of 8-inch diameter, 10-inch diameter, and 12-inch diameter culinary water lines. No new connections were added as part of this project. However, the new Lower Tank RPL-1 (ST003) will be located at a higher elevation than the previous Lower Tank (ST002) and will create a new pressure zone. New pipelines will be installed from the existing Upper Tank (ST001) to the town’s existing Upper PRV Station, then from the Upper PRV Station to the town’s existing distribution system, and from the Lower Tank RPL-1 (ST003) to the town’s existing distribution system. We have received the following information for the Lower Tank RPL-1 (ST003), new well pump for Town Well (WS002), and water line replacements for UTAH26009 Distribution System (DS001): Certification of Rule Conformance with plan approval conditions by a professional engineer Caleb Dotten, P.E. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Lower Tank RPL-1 (ST003), new well pump for Town Well (WS002), and water line replacements for UTAH26009 Distribution System (DS001) is hereby issued as constituted by this letter. You may now place the facilities in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.
