Notes:The line is disconnected so the piping is exposed. I let them know they need to screen it or
connect it.

Rule: R309-545-6(1) and 545-9 Rule requires openings in a storage tank roof or top, designed to accommodate control
apparatus or pump columns, to be welded, gasketed, or curbed and sleeved and to have
additional proper shielding to prevent vandalism. This significant deficiency must be
corrected within 120 days of notification or have a corrective action plan approved by
DDW.
Notes:The SCADA junction box on the west side of the roof is broken and needs to be replaced. A
mouse could possible access the opening and get into the tank.
