INCIDENT SUMMARY
Caller reported that during a diesel fuel transfer from a tanker truck to an above-ground storage tank, the tank overfilled and approximately 75 gallons of
diesel fuel spilled. Most of the diesel spilled on the concrete and approximately 30 gallons spilled on the soil. No storm drains were impacted.
Clean-up on the soil was being performed this morning with a skid steer and the contaminated soil will be properly disposed of. Environmental
Restoration is scheduled to take soil samples on Tuesday 1/21.

On Tuesday, January 21st, 2025, Environmental Restoration, LLC received a request for emergency response actions
in North Salt Lake, UT. The following information was provided at time of request:

Upon receiving the response request, Environmental Restoration, LLC immediately began coordinating the appropriate
personnel and equipment regarding the above referenced project.
ERLLC is contacted by Tessah Chappelow with Reladyne regarding a 75-gallon diesel release that they had last week.
Reladyne has performed the remediation themselves, but they need sampling done at the site to ensure the site is free of
contamination. She has also requested that the 1-yard of diesel impacted soil that was generated is profiled in for disposal.
Reladyne will transport the soil to the TSDF once it is approved.
ERLLC SLC will be sending an RM to the site to perform the sampling
ERLLC begins profiling the soil into a local landfill.
Thursday, January 23rd, 2025
The RM mobilized the site to inspect the site and take photographs. Reladyne was provided with an update, and they want
to hold off on sampling for the time being. They have requested that ERLLC perform an excavation at the site. The RM then
returned to his office and fueled up his vehicle.
Wednesday, January 29th, 2025
ERLLC received the profile approval. A manifest was generated, and a team member delivered the manifest to personnel at
Reladyne's office in North Salt Lake. Personnel will be able to deliver the waste to the TSDF any time after today. Team
members will be responding to the incident site tomorrow to perform the excavation. A state-approved contractor will
accompany the team to take samples at the site.
Thursday, January 30th, 2025
ERLLC SLC performed the excavation at the site. The team was accompanied by a state approved contractor who is going
to be taking samples from the site. The team averaged a depth of 6 inches during the dig. The impacted soil was loaded into
two roll off boxes. The contractor used a meter to determine that the diesel had been removed and then proceeded to take
his samples. The team will return to backfill the site pending the results from the lab.
Friday, February 7th, 2025
ERLLC SLC transported the roll off box of contaminated soil to the TSDF.
Tuesday, February 11th, 2025
ERLLC SLC returned to the site to backfill. The analytical results arrived earlier and indicated there is no more
contamination. Once the site was backfilled, team members compacted the area. The landowner inspected and approved
the work area. Team members then returned to their office.
