Malfunction for the reporting period of January 1, 2024 through June 30, 2024.

compliance with the reporting requirements associated with the Maximum Achievable Control
Technology (MACT) regulations identified in Subpart EEE.

affected source (inctuding actions taken to correct a malfunction) are consistent with the
procedures specified in the source's Startup, Shutdown, and Malfunction Plan (SSMP), the
owner or operator shall state such information in a startup, shutdown, and malfunction report.
During the reporting period the facility was consistent with the operating procedures specified
in the Startup, Shutdown, and Malfunction Plan.

On May 6,2024, after the first day of the Confirmatory Performance
Test (CfPT), the stack testers compared the flow measurements from
Tooele Army Depot's (TEAD's) stack gas velocity meter. TEAD's meter.
was reading significantly lower at 35 fps vs the stack testers of 50
