ln accordance with the Operating Permit provision l.S.2.c and Utah Administrative
Code (UAC) R307-415-6a(3)9(c)(ii), the following deviation report is being submitted:
a. The dates the deviation occurred:
- 51612024
b. Permit condition for which the deviation occurred:
- ll.B.l.e.1 condition, of the Title V Permit
c. Describe the deviation that occurred:
- On May 6,2024, after the first day of the Confirmatory Performance Test (CfPT),
the stack testers compared the flow measurements from Tooele Army Depot's (TEAD's)
stack gas velocity meter. TEAD's meter was reading significantly lower at 35 fps vs the
stack testers of 50 fps causing an instrumentation failure.
d. Was the deviation due to an "upset condition" as defined in of the permit?
- No.
e. Was the deviation due to an unavoidable breakdown as defined in UAC R307-
107?
- No.

f. Describe the probable cause of the deviation:
- This was due to instrumentation failure.
g. Describe the conective action or preventive measures taken:
- Personnel replaced the flow meter and continued the (CfPT) testing. The
preliminary results show TEAD's stack flow meter had passed the RATA.

based on information and belief formed after reasonable inquiry, I certify that the
statements and information in this document are true, accurate and complete.
