Dear Blake Anderson:The Division of Drinking Water (the Division) received a request for an Operating Permit for the water main replacement project from your staff Kris Nilsen, P.E.
S.E., on January 21, 2025. As-built drawings were received on January 17, 2025. Bacteriological sample results were received on February 12, 2025. The Division issued Plan Approval for
this project on April 20, 2023.
Our understanding of the project was replacing approximately 1,750 linear feet of the existing water main with a new 8-inch diameter water main. The sections being replaced were the
560 West cul-de-sac, 1890 North starting at 600 West, through the West Bountiful City Park parking lot, then 1810 West returning to 600 West. The replacement along the 560 West cul-de-sac
was already granted a separation exception under Plan Review File #13280.
We have received the following information for the Water Main Replacement Project:
Certification of Rule Conformance with plan approval conditions by a professional engineer Kris Nilsen, P.E., S.E., dated January 10, 2025
As-built drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Water Main Replacement Projectis hereby issued as constituted
by this letter.
