Box 277 Midway, UT 84049 Subject:Plan Approval and Exception to R309-550-7(1) Minimum Separation Requirements, North Center Street Water Line Replacement Project, Utah26008 Distribution System (DS001); Midway City Water System, System #26008, File #18918 Dear Shane Owens:The Division of Drinking Water (the Division) received the plans and specifications for the North Center Street Water Line Replacement project from your consultant, Derek Anderson, P.E. with Sunrise Engineering, on January 14, 2025. This project has met the conditions for receiving a plan approval. On January 23, 2025, the Division received a completed Water/Sewer Separation - Exception Request Form and Water/Sewer Separation - Exception Request Spreadsheetfor an exception to Rule R309-550-7(1), Separation of Water Mains and Transmission Lines from Sewers and Other Pollution Sources for Utah26008 Distribution System (DS001). This rule states specifically that the horizontal distance between pressure water mains and sanitary sewer lines shall be at least ten feet and that where a main and a sewer line must cross, the water main shall be at least 18 inches above the sewer line. Plan Approval Our understanding of the project includes replacing approximately 3,745 linear feet of existing 8-inch water line to include the installation of 3,500 linear feet of 10-inch diameter PVC C900 DR-18 and 245 linear feet of 8-inch diameter PVC C900 DR-18 along North Center Street from 600 North to Main Street and along Alfalfa Circle. The work includes replacing existing service connections and associated appurtenances. We have completed our review of the plans and specifications, stamped and signed byJohn Keyser, P.E., dated December 2, 2024, and find they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for the North Center Street Water Line Replacement project are hereby approved. This approval pertains to construction only. An Operating Permit must be obtained from the Director before the water lines may be put in service. A checklist outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, a notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter. Exception to R309-550(7)(1) – Minimum Separation Requirements The background and basis of your exception request to R309-550-7(1) is included in the attached Water/Sewer Separation - Exception Request Form and Water/Sewer Separation - Exception Request Spreadsheet. On the basis of the submitted documents, an exception to R309-550-7(1), Separation of Water Mains and Transmission Lines from Sewers and Other Pollution Sources, is hereby granted for the North Center Street Water Line Replacement project at the locations included in the attached form.This exception may be rescinded any time if new evidence indicates that the conditions of this exception are no longer being met or a public health risk exists due to this exception. Please maintain a copy of this letter with your permanent records for future reference.
