In order to ensure that all of the sources in an entry point grouping are active at the time
of sampling the water system must submit to DDW a sampling plan that shows the sources in the group source sampling station, their operational periods, and planned dates of sampling. If a source for a particular entry point goes inactive for more than one year the system will notify DDW.
The Division of Drinking Water will review the entry point submittal and decide if the system qualifies for entry point sampling. To meet this requirement please:
1. Provide a description of how the sources combine, including which facilities they
flow through. 2. Fill out the following table. 3. Sign and date the certification statement.
Flow Description:
Wsoo1 well andwsoo2 combind and both feed in to a singly line out let toasing WS001 and WS002 combine into a common tank prior to any service connections. Samples will be pulled at the outflow of the tank.

Example:
Well #2 (WS001), Well #6 (WS002) and New Well (WS005) are pumped into the 500k tank (ST001) prior to entry into the distribution system. The system is configured in a way to where water from each well has to flow into the storage tank before it enters into the distribution system.
