Hey Olivia,
I am looking for information on the diesel release in Herriman, UT, on 8/3/2025. Please submit a written 15-day report with
the information seen below, as outlined in Utah Admin Code R315-263-33. This report is required to close out incident
number 19209 (attached). Let me know if you have any questions.
(a) The person's name, address, and telephone number;
(b) Date, time, location, and nature of the incident;
(c) Name and quantity of material(s) involved;

(e) An assessment of actual or potential hazards to human health or the environment, where this is applicable; and
(f) The estimated quantity and disposition of recovered material that resulted from the incident.

Spill Date & Time: 8/3/2024 @ 6:00 am
D9 saddle tank punctured and released 85-90 gallons of diesel fuel onto surrounding soils.
There were no injuries and no potential hazards to human health or the environment. It was cleaned up
immediately.
Material recovered from the site was taken to Staker Parsons/Reynolds Pit in West Valley until it was hauled
to the Clean Harbors Grassy Mountain Facility for final disposal.
