Wasatch Environmental, Inc., (Wasatch) has completed confirmation sampling following soil remediation
activities at the Signature Aviation FOB fuel farm (Site) located at 3421 Mike Jense Parkway, in Provo,
Utah (see Figure 1).
The remedial activities involved the removal of Jet A fuel impacted soils around and beneath
aboveground storage tanks (ASTs) #1 and #2. The released Jet A fuel and fuel-impacted soils have
been remediated. Confirmation soil samples were collected following the soil removal activities. All soil
confirmation sample results were below the Utah Division of Environmental Response and Remediation
(DERR) Risk-Based Corrective Action (RBCA) Tier 1 Screening Levels with the exception of one sample
collected from the southeast corner of the excavation which exceeded the Utah RBCA Tier 1 Screening
Level for Total Petroleum Hydrocarbons as Diesel Range Organics (TPH-DRO). Additional discussion of
this sample and probable matrix interference from asphalt is discussed in the Conclusions and Discussion
section of this report. Based on the totality of the data collected and lack of potential receptors, this report
concludes that the Site remediation is complete, and that release ID OMF should be closed.
BACKGROUND
On December 17, 2024, an overfill of Tank 2 occurred during fuel delivery resulting in a release of
approximately 300 to 400 gallons of Jet A fuel from the vent pipe and onto the tank and surrounding
graveled surface beneath the tank as well as spreading to areas beneath Tank 1, located to the south
(Figure 2).
Signature Aviation requested that Wasatch provide a Utah Certified underground storage tank (UST)
Consultant to provide professional oversight and direction of the cleanup activities, provide regulatory
support with the Utah DERR, collect the required confirmation samples for submittal for laboratory
analysis, and prepare a report to document that the release has been properly addressed in order to
achieve regulatory closure from the Utah DERR.
Discussions with DERR Project managers indicated that based on land use and distance to receptors
being greater than 30 feet, the RBCA Tier 1 Screening levels would be an appropriate cleanup target
concentration. Utah RBCA Tier 1 Screening Levels are generally applicable to sites where petroleum
impacts are more than 30 feet from receptors (i.e., utilities, buildings, and property boundaries, etc.) and
where the land use is commercial or industrial. Additionally, there can be no water wells or surface water
bodies within 500 feet of the release.
The nearest navigable water to the facility is Utah Lake which adjoins the western boundary of the Provo
Municipal Airport (PVU). The tank farm is located approximately 0.5 miles east of the lake.

Drainage improvements within the limits of the PVU result in the collection, detention, and discharge of
stormwater generated from the airport property, including the Signature Aviation operating areas to a
perimeter drainage detention canal. Discharges from this man-made detention canal into Utah Lake are
controlled by a pumping station that is monitored as part of the requirements of a Utah Pollution
Discharge Elimination System (UPDES) general permit for the City of Provo. The canal does not flow,
and active pumping is not employed unless stormwater has collected in the detention canal. The
presence of a detention canal, identified in the SPCC Plan as a physical spill control measure, would
disqualify the canal as a receptor. During remediation, no sheen was observed in the detention canal.
The closest water well identified on the Utah Division of Water Rights map is located 610 feet west of the
release site. It is water right 55-3888, owned by Provo City. It is not known if this well, drilled in 1910, is
still in use. Potable water and wastewater services in the area are provided by Provo City Corporation.
REMEDIAL ACTIVITIES
Environmental Restoration, LLC (ER) conducted impacted soil removal activities during three remediation
events between December 19, 2024, and January 6, 2025. ER loaded the excavated soil into six covered
roll-off boxes staged north of the tank farm. The excavation was guided largely by field observations of
surface staining, the presence of degraded asphalt, and fuel odor observed in soils. Additionally, soil was
placed into resealable plastic bags and warmed up inside a vehicle before screening with a MiniRae 3000
photoionization detector (PID) equipped with a 10.6 electronvolt lamp. Additionally, soil was screened for
the presence of total petroleum hydrocarbons (TPH) using Cheiron Resources, Ltd., Oil Screen for Soil.
The final excavation limits and soil confirmation sample locations are presented on Figure 2.
Signature Aviation profiled the waste stream at the Republic Industries Wasatch Regional Landfill (WRL)
near Skull Valley Utah, prior to disposal. From January 23 through February 6, 2025, six roll-off bins of
impacted soil were transported to the WRL for disposal. The total weight of soil generated and disposed
of was 86.3 tons. Groundwater was not encountered during excavation activities. Copies of the non-
hazardous waste manifests and weigh tickets are presented in Appendix A.
CONFIRMATION SAMPLES
After each soil remediation event, Wasatch collected confirmation soil samples from the floor and
sidewalls of the excavation. Once the analytical results were received and reviewed, additional
remediation of soil was planned for those areas which did not meet the Utah RBCA Tier 1 Screening
Levels. The confirmation soil sampling occurred on the following dates:
• December 21, 2024 – Nine confirmation samples were collected,
• December 30, 2024 – Four confirmation were collected, and
• January 6, 2025 - Three confirmation samples and one extent of impact (Test Pit 1) sample were
collected.
Soil sample locations are depicted on Figures 1 and 2. All confirmation soil samples were collected by
Wasatch Senior Geologist, Christopher Nolan (Utah UST Groundwater and Soil Sampler GS 1101) with
gloved hands using a decontaminated trowel and placed in appropriate laboratory-supplied sample jars.
The samples were transported in an iced cooler, under chain-of-custody protocol, to Chemtech-Ford
Laboratories, a Utah-certified analytical laboratory in Sandy, Utah.
The samples were analyzed for TPH-DRO full scan by the United States Environmental Protection
Agency (U.S. EPA) Method 8015, with silica gel treatment (SGT) and methyl-tert butyl ether (MTBE),
benzene, ethylbenzene, naphthalene, toluene, and total xylenes (MBTEXN) and total petroleum

hydrocarbons as gasoline range organics (TPH-GRO) by U.S. EPA Method 8260D. In addition, selected
samples were also sampled for petroleum fractionation for volatile organic compounds and semi-volatile
organic compounds (SVOCs) by U.S. EPA Method 8260D, and by U.S. EPA Method 8270E, respectively.
At the request of the Utah DERR project manager, Wasatch collected a soil sample between the
perimeter drainage detention canal and the release site. The sample location was 30 feet northeast of the
Northeast fence corner as shown on Figure 1 at a depth of 2.5 feet below ground surface (bgs). It is
incorrectly labelled as being collected at 2.5 inches on the laboratory report. The Test Pit could not be
advanced directly north of the fuel farm as that area was underlain by asphalt. The sample results from
location Test Pit @ 2.5’ were non-detect for all analytes. No staining or petroleum odors were observed
in the test pit. Sample locations for the remaining confirmation soil sampling event are shown on Figure
2.
Table 1 summarizes the analytical results from the confirmation soil samples. For comparison purposes,
the Utah DERR Initial Screening Levels (ISLs) and Utah RBCA Tier 1 Screening Levels are also
presented on Table 1.
All laboratory quality control/quality assurance protocols were achieved. The laboratory analytical
reports, including chain-of-custody documentation, are presented in Appendix B.
CONCLUSIONS AND DISCUSSION
The analytical results of confirmation soil sampling indicate that after the excavation of Jet A fuel-
impacted soil was completed, the soil remaining at the release site was below the Utah RBCA Tier 1
Screening Levels, with the exception of sidewall sample @ OX Wall @ 1.5’.
Sample OX Wall @ 1.5’ has a TPH-DRO concentration of 5,900 milligrams per kilogram (mg/kg) which
exceeds its Utah RBCA Tier 1 Screening Level of 5,000 mg/kg. It is Wasatch’s opinion that this elevated
TPH-DRO concentration was the result of leaching from the asphalt and asphalt millings which were
present around and below tank #1 and #2. The basis for this assertion was observed in a confirmation
soil samples Overex Wall W @ 1.5’ and Overex Wall E @ 1.75’ which had very elevated TPH-DRO
analytical results. However, the field screening did not indicate a strong fuel odor, the PID measurements
were around 22 parts per million (ppm), and the TPH field screening kit indicated that total TPH was
below 500 ppm. The TPH screening test does not respond to bituminous asphalt components.
On January 6, 2025, Wasatch collected a sample of soil approximately 0.5 feet below an area of
degraded asphalt and submitted it to Pace National Laboratories to “fingerprint” the types of TPH the soil
contained. A comparison of the chromatograph from the sample with a standard chromatograph of turbine
fuel (Jet A) shows additional hydrocarbons present in the area of the chromatograph beyond the 4.25-
minute mark. These peaks represent petroleum hydrocarbons not related to Jet A fuel. Pace
Laboratories postulated that the Jet A fuel acted as a solvent to dissolve the asphalt hydrocarbons and
were identified in the TPH-DRO analysis as carbon lengths C-10 to C-28. The results of the analysis are
presented in Appendix C.
The analytical results of sample 2 OX Wall E @ 1.5’ were entered into the Utah DERR Site Specific
Cleanup Level (SSCL) calculator spreadsheet. As presented in Appendix C, the only completed receptor
pathway is for alkyl benzenes for groundwater ingestion. Shallow groundwater in the Provo airport area
is not potable and not used. The SSCL spreadsheet is attached in Appendix C.
The soil excavation at the Signature AST Jet A release was backfilled with clean fill and the ASTs are in a
restricted area with a locked fence. Therefore, no potential soil impacts are present at the ground surface
and the excavated area has been restored to a condition that would be protective of human health and
the environment. Wasatch recommends that the release be granted regulatory closure.

CASE NARRATIVE
All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form
or properly qualified within the sample results. By my digital signature below, I affirm to the best of my
knowledge, all problems/anomalies observed by the laboratory as having the potential to affect the
quality of the data have been identified by the laboratory, and no information or data have been

Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
J3 The associated batch QC was outside the established quality control range for precision.
J7 Surrogate recovery cannot be used for control limit evaluation due to dilution.
V The sample concentration is too high to evaluate accurate spike recoveries.
