Subject:Operating Permit, Farr West Well Upgrades (WS003);
Bona Vista Water District, System #29004, File #13429
Dear Blake Carlin:The Division of Drinking Water (the Division) received your request for the Operating Permit for Farr West Well Upgrades from you on March 19, 2024. The Division issued
Plan Approval for equipping this well on July 26, 2023. Division staff conducted an onsite final inspection of this facility on March 21, 2024.
Our understanding of the project is the complete replacement of the existing 100 HP motor and
pump with a 200 HP motor and pump. This included the replacement of the sole plate, discharge
head, sounding tube, power connections, and all other necessary appurtenances. The pump bowls
were lowered from 150 feet to 300 feet. Flow capacity will increase from approximately 820
gallons per minute (gpm) to 1,200 gpm. Other well facility improvements included the installation
of a backup power generator and upsizing of some of the mechanical piping. This well is
identified as WS003 in the Division’s database.
We have received the following information for Farr West Well Upgrades:
Confirmation of conformance with approval conditions.
Evidence of O&M manual delivery.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Satisfactory nitrate results.
We have determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit forFarr West Well Upgradesis hereby issued as constituted by this
letter. You may now place Farr West Well (WS003) in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
The equipped well pump capacity of Farr West Well is 1,200 gpm. The safe yield of Farr West Well is rated at 800 gpm, which is calculated based on two-thirds of the constant-rate aquifer
drawdown test results at 1,200 gpm. The safe yield of 800 gpm is the basis for determining the maximum number of connections that Farr West Well can serve.
Issuance of this Operating Permit changes your water system’s monitoring and reporting
requirements. The monitoring requirements for this well are given below. An updated monitoring schedule can be viewed at any time at waterlink.utah.gov under the Water Monitoring section.
