Box 26210 Oklahoma City, OK 73126 Incident Description: Diesel Fuel Release Incident Time/Date: November 29, 2024/ 0750ET Incident Location: Love's Travel-Travel Stop 792 1810 Main Street Green River, UT 84525 Emery County Utah DEQ #: 19401 ERTS Project # 222719 To Whom it May Concern, The following serves as the final report detailing the emergency response and corrective actions taken in response to the incident that occurred on November 29, 2024, at the above-referenced location. Please see the site map (Appendix 1). Incident Background: On November 29, 2024, at 0750 ET, Gemini Motor Transport encountered a diesel fuel release during fuel delivery operations due to a reported device failure. As a result, approximately 50 gallons of diesel fuel were released, impacting the concrete pad and parking lot. No soil, drains, or waterways were reported to be affected by the release. Emergency Response: Emergency Response and Training Solutions ERTS 11231 Phillips Industrial Blvd. Bldg. 1, Ste 300 ● Jacksonville, FL 32256 ● USA phone number: 440.349.2700 | fax number: 440.349.6713 ertsonline.com On November 29, 2024, at 0753 ET, Love’s Travel Stops and Country Stores retained Emergency Response and Training Solutions (ERTS) to manage and oversee environmental operations. ERTS dispatched Enviro Care, Inc out of Duchesne, UT to conduct all onsite cleanup and remediation activities. Regulatory Notifications: Pursuant to Utah Administrative Code, R315, Rules 315-9-1, 315-9-4, on November 29, 2024, at 1214 ET, ERTS contacted the Utah Department of Environmental Quality (DEQ). Operator Nicole Chavez received the call; report number 19401 was assigned to this incident. Corrective Action: On November 29, 2024, at 1320 ET, Enviro Care personnel arrived on-site and conducted an initial assessment. They reported that an estimated 100 gallons of diesel fuel was released, affecting an area measuring 250 feet by 30 feet on the concrete slab. The fuel migrated into the parking lot, impacting a nearby drain basin. Enviro Care confirmed that no material flowed beyond the drain basin. The team then began operational procedures, which included pressure washing the affected asphalt. They used a vacuum truck to collect the rinse water and recover the impacted water from the drain basin. Enviro Care collected 359 gallons of diesel-impacted water and removed the waste from the site for pending disposal. Please see the attached photos provided by the contractor (Appendix 2). Waste Disposal: On December 3, 2024, 359 gallons of non-hazardous, diesel-impacted water was transported to Sump & Trap Cleaning in Salt Lake City, UT, for final disposal. Please see the disposal document (Appendix 3). Conclusions and Recommendations: No evidence suggests any environmental impact remains on the site following corrective action. ERTS recommends that the incident be closed. Love’s Travel Stops are working to replace the overfill flapper or install an overfill system at the site to prevent future incidents.
