Dear Mr. Jenkins:
The Division of Environmental Response and Remediation (DERR) received a revised
Work Plan and Budget (WP&B) on September 23, 2024. The WP&B dated September 12, 2024,
was prepared by Pure Enviro Management, LLC. The WP&B is for additional subsurface
investigation, the replacement of seven on-site wells, and site restoration.
As per discussions with your consultant, Pure Environmental, there is insufficient
information included in the WP&B to assess costs that may be improvements to the property and
which have been included as part of site restoration. Please resubmit a revised WP&B for this work,
including an itemized Bid Specification and Schedule, by February 10, 2025. A sample Bid
Specification and Schedule has been provided as an example of items reimbursable by the fund, but
it is not intended to cover all project requirements. Your engineer should prepare a comprehensive
Bid Specification and Schedule for this project with reimbursable costs outlined in the example
broken out separately.
Pursuant to Utah Admin. Code R311-207-4(8), you are required to obtain a minimum of
three competitive bids by responsive bidders, or you must provide an acceptable sole source
justification. In order to assist you in obtaining competitive bids, please see the attached list of local
contractors. Also, previously submitted WP&Bs included bids that combined projects that included
costs to be covered by the owner and costs to be covered by the Petroleum Storage Tank Fund (PST
Fund).

Please separate out the work and costs to be covered by the PST Fund for the DERR review.
If bidders cannot or will not separate costs in this manner, you may need to obtain separate bids for
PST Fund covered work.
In accordance with the Cost Guidelines for Utah Underground Storage Tank Sites, costs of
upgrading or improving a site are ineligible for reimbursement from the PST Fund. This includes
costs associated with redevelopment to meet state and local permitting and code requirements, such
as upgrades to structures, curb, gutters, and stormwater drainage, and will remain the responsibility
of the property owner. Additionally, the PST Fund cannot cover an improvement that was not
present before the remediation project began and bids should clearly show the work being done so
a determination of costs that can be reimbursed are easily distinguishable from those that are not
reimbursable.

UNIT PRICES
Contractor shall complete the portion of the Bid Schedule for listing unit prices, inserting its unit
Bid price for each item. The unit Bid price times the quantity determines the total price for each
Bid item. The summation of total Bid prices results in the Contract Total Price. In the case of an
error in extension, the unit price shall govern. Failure to enter a price for any Bid item will cause
the entire Bid to be thrown out as non-responsive.
Unit prices shall include all Contractor’s costs, expenses, general conditions, overhead, labor,
material, supervision, tools, equipment, insurance, bond premiums, taxes, fringe benefits,
coordination and all other necessary costs, including the Contractor’s profit.
Unit prices shall not increase by any change in material prices, wage rates or any other escalatory
factors and shall remain in effect through the duration of the Remediation Project unless otherwise
stated.
The Work shall be furnished and installed complete in every respect or deleted from the Work in
accordance with the Bid Schedule listed. Unit prices shall be based upon all Work complete in
place in accordance with all applicable requirements of the Contract Documents.

This letter is to inform you that the Drainage Plan for your project has been reviewed and found to be in
conformity with Salt Lake County Standards. Prior to receiving FINAL APPROVAL from Engineering, it is
necessary for you to sign the Urban Hydrology Requirements Agreement and pay for all remaining Engineering
fees. A draft copy of the document is provided for your review and comment. Within the Urban Hydrology
Requirements Agreement are: a summary of the outstanding engineering fees that are due at this time, the
hydrology requirements for the project (see Attachment A), and the calculations for the Final Engineering Review
When the above items are complete, your approval will be forwarded to Planning Services. Please be advised that
Salt Lake County Ordinances require payment of all fees and posting of bonds prior to the approval of subdivision
plats or issuance of building permits. A copy of the Engineering bond calculation has also been provided with this
letter for your reference. This bond amount will be included in the overall bond calculation for the bond
agreement document, which the County will prepare for you.

Urban Hydrology approval of the drainage system proposed for this application is based upon the engineering review
of the drainage and grading plans and the resulting requirements on the proceeding ATTACHMENT A. The
requirements listed, together with the final plans, comprise the drainage approval. When infrastructure costs are
involved, the applicant may be required to post a performance bond as a condition of approval.
This development is subject to Title XVII, Chapter 17.20, of the revised Salt Lake County Ordinance which pertains
to payment of storm drainage impact fees. Those fees and/or bonds which may appear below are in conformity with
this Ordinance and the attached assessment data sheet (Attachment B). All fees must be paid and bonds posted (as
required) prior to the approval of subdivision plats or issuance of building permits. Fee and bond amounts appear on
FINAL approvals only.

All public storm drain facilities and private facilities which directly connect to the public storm drain system
or facility are required to have a pipe condition assessment performed using CCTV per Section 33 08 00 of
the current Manual of Standard Specifications from APWA.

The developer, his engineer and contractor shall meet with Salt Lake County inspectors two days prior to
commencing construction of any irrigation or drainage systems. Call Ron Coshow at (801) 833-9819 to
coordinate this meeting. The inspector will have the developer's copy of the approved site plans.
Prior to any excavation or construction within an existing county right-of-way, the developer shall obtain an
"Excavation Permit" as per Chapter 14.16 of the Revised Ordinances of Salt Lake County. Contact
Engineering and Flood Control Services at (385) 468-6600 for more information.
The developer shall be responsible for all costs incurred due to utility conflicts. Any design modifications
resulting from utility conflicts must be submitted to Greater Salt Lake Municipal Services District and
approved in writing by the Engineering Division prior to acceptance of improvements.
The developer shall grade the property in accordance with the approved site grading and lot drainage plan so
as to direct storm water to an approved public facility or to permanently retain all generated water on the
subject property both during and after construction.
All storm water generated from the project shall be maintained so as not to discharge any additional storm
water onto adjacent properties or pond at the end of the curb and gutter.
All outstanding balances for the 3rd Party Review Fee, Engineering Checking Fee, and Bond Processing Fee
shall be paid at the Planning and Development Services Office prior to the approval of subdivision plat or
issuance of building permit.
A performance bond for this project shall be posted and processed through the Development Services Office
prior to the approval of subdivision plat or issuance of building permit. For additional information, please

The drainage system designed by Matthew Kimball Smith, P.E., License No. 6288195, and stamped
'Approved' by Greater Salt Lake Municipal Services District shall be constructed as shown. Any changes,
additions or deletions to the approved plans must be submitted to Greater Salt Lake Municipal Services
District and approved in writing by the Engineering Division prior to commencing construction.
Reinforced concrete pipe sizes referred to herein are based on class 3, rubber gasket reinforced concrete
pipe, and shall have a minimum of one-foot of cover. Cleanouts shall be at maximum 400-foot intervals.
Two feet of fill shall be placed over the crown of the reinforced concrete pipe during construction activities
to protect against extremely high loads created by construction vehicles. The fill shall be graded to final
grade with minimum one-foot of cover over pipe after heavy equipment has left the project.
All catch basins and cleanout boxes shall be constructed to County standards. See Salt Lake County Public
Works Standard Plans for details. All boxes shall be poured in place in accordance with the above standard
drawings.
Installation of pre-cast boxes is allowed in private storm drain and irrigation systems that are not within the
public right-of-way or within development access roads.

All cleanout boxes and catch basins that are part of a public storm drain system or within the public right-of-
way shall conform to the current Standard Plans for Public Works Construction , published by Salt Lake
County. Cleanout boxes and catch basins shall be inspected in the field by Salt Lake County inspectors
prior to pouring concrete. Pre-cast boxes are not allowed except by Salt Lake County Certified
manufacturers.
Salt Lake County does not assume any ownership or liability for private storm drain or irrigation systems.
The Developer shall assume all responsibility for the maintenance, operation and safety of private storm
drain and irrigation systems associated with the project.
Bonds on detention basins will not be released until a certified as-built survey is received and approved by
this Division.
