Dear Mr. Cahoon:
The file for this release has been reviewed by the Division of Environmental Response and
Remediation (DERR) project manager, who has recommended that no further corrective action be
taken at this time. This no further action applies only to the specific Release Site OMH, reported to
the DERR on January 2, 2025. This recommendation is based on information contained in the file
for the release. That information was supplied by you, your consultant, or another representative.
The information submitted indicates that any detectable petroleum contamination from this
release is not a threat to human health or the environment, according to Utah Admin. Code rule
R311-211. In the future, if other evidence indicates the presence of contamination from this release
which may cause a threat, further corrective action may be required.

SUBJECT: No Further Action: Surface Spill, Leaking Petroleum Storage Tank Release
Holiday Oil #54, located at 578 South Pleasant Grove Blvd, Pleasant Grove, Utah
Facility Identification No. 1000914, Release Site OMH
I have reviewed the information for the above-referenced release, which includes information provided by the
facility owner. My recommendation that no further action is required for the release is based on the review
of the information contained in the case file, after due consideration of the corrective action clean-up standards
policy (Utah Admin. Code R311-211.
Case file information justifying no further action for the release includes the following:
1) Seven gallons of gasoline was released to the concrete surface on January 3, 2025, when a customer
overfilled their tank. Two gallons entered the dispenser-area drain system which eventually connects
to the sanitary sewer line south of the dispensers.
2) Absorbents were used on the surface area of the spill.
3) Sump & Trap pressure washed the area and the impacted portion of the drain system. 120 gallons of
water, mud, debris and gasoline were disposed at Sump & Trap in Salt Lake City.
4) No gasoline reached the sanitary sewer line. There was no impact to the subsurface.
No Further Action is recommended for the release. Release data indicate no significant petroleum impacts
exist or remain on-site and do not appear to constitute a current or potential threat to human health or the
environment. This recommendation for close-out is based on the condition that if future evidence indicates
the existence of contamination at or emanating from this site, additional investigation and corrective action
may be required. A No Further Action letter will be sent to the responsible party.
