Your Utah Waste Tire Transporter Registration Application has been reviewed and approval for registration as a waste tire transporter is hereby granted. Great Salt Lake Recycling, LLC
has been assigned the UtahWaste Tire TransporterNumber A-157208.Great Salt Lake Recycling, LLCshould use this number on all future correspondence sent to the Division of Waste Management
and Radiation Control (Division) regarding the transporting of waste tires in the State of Utah.
This registration as a waste tire transporter will be valid until December 31, 2025.
Quarterly Activity Reports are to be submitted to the Director on or before the 30th of the month following the end of each quarter as required by Utah Administrative Code R315-320-4(9).

This approval is required to comply with requirements of theWaste Tire Recycling Act in the State of Utah. Maintenance of a valid registration and full compliance with all applicable
requirements for a waste tire transporter does not relieve waste tire transporters of the responsibility to obtain and comply with any other required local, state, or federal approvals
or permits. This registration may be revoked in accordance with Utah Admin. Code R315-320-4(10).
