Part I Instructions Section I – Type of Waste Tire Pile to be Removed Check the appropriate box. This application is to be used by state and local government landfills and transfer stations, or by a county or municipality applying for an abandoned waste tire pile cleanup. This application, with approval from the Division to complete the removal, is required to receive reimbursement. Reimbursement is only applicable for a landfill or transfer station located in a county of the first or second class (a population of 175,000 or more). Only state and local government landfills or transfer stations can receive a reimbursement for the removal of waste tire piles from a landfill or transfer station, and delivery to a registered waste tire recycler. A reimbursement can only be issued if proper approval is granted by the Division to pursue the desired waste tire pile removal as designated in UCA 19-6-811(1) through 19-6-811(5). A Waste Tire Pile is defined in UCA 19-6-803(3): "Waste tire pile" means a pile of 200 or more waste tires at one location. An Abandoned Waste Tire Pile is defined in UCA 19-6-803(1): Page 1 of 6 "Abandoned waste tire pile" means a waste tire pile regarding which the local department of health has not been able to: (a) locate the persons responsible for the tire pile; or (b) cause the persons responsible for the tire pile to remove the tire pile. Section II – Does the Waste Tire Pile Contain More than 200 Waste Tires? Check the appropriate box. To be applicable for a reimbursement to remove a waste tire pile, the pile must meet the definition of a waste tire pile. A Waste Tire Pile is defined in UCA 19-6-803(3): "Waste tire pile" means a pile of 200 or more waste tires at one location. Section III - Location of Waste Tire Pile Record the exact location of the waste tire pile. If the location has a physical address, please include it. If there is no physical address, include cross streets and/or directions to the site. If the removal is taking place at a landfill, include the name of the landfill under NAME OF FACILITY. Section IV - Facility Operator Information Record the individual, governmental entity, or other legal entity that provides daily operations at the facility. Section V - Registered Transporter / Recycler Information (1st Bid) The person listed as CONTACT NAME AND TITLE should be the individual that the Division can communicate with for general information and questions about the removal request. The CERTIFICATE NUMBER is the registration provided by the Division confirming the transporter/recycler is currently registered. Please contact the transporter/recycler to obtain their Certificate Number. Section VI - Registered Transporter / Recycler Information (2nd Bid) The person listed as CONTACT NAME AND TITLE should be the individual that the Division can communicate with for general information and questions about the removal request. Page 2 of 6 Section VII - Registered Recycler Information (locations waste tires will be delivered to) The person listed as CONTACT NAME AND TITLE should be the individual that the Division can communicate with for general information and questions about the reimbursement process. The applicant’s address is the business mailing address. The CERTIFICATE NUMBER is the registration provided by the Division confirming the transporter/recycler is currently registered. Please contact the transporter/recycler to obtain their Certificate Number. Part II Additional Information Section I - Required Supplemental Documents The individual requesting a removal must supply the applicable documents in this section to be considered a complete application. The recycler, waste tire transporter, or landfill that is requesting removal shall include: (a) Local Health Department (LHD) Statement Certifying Abandoned WasteTire Pile Status (if abandoned) (b) Copies of Bids to Remove Tire Pile (c) Cost Breakdown to Remove Landfill Tire Pile (if not using a 3rd party) (d) Waste Receipt Records (if this is a landfill/transfer station waste tire pile removal) Not all of the required supplemental documents will be applicable to every situation. If you are applying as a county or municipality to remove an abandoned tire pile, this application is required to include only (a) and (b). If you are a landfill or transfer station requesting removal and using a registered waste tire recycler or registered waste tire transporter, this application is required to include (b), (c), and (d). Section II – Amount of Tires Fill in the amount of tires (number of tires or total weight) which are to be removed from the waste tire pile. The weight entered in this section needs to match the landfill or transfer station’s waste receipt records if this is not an abandoned waste tire pile. Authorized Signature The applicant submitting this form for waste tire removal must sign here. An application submitted with no signature will not be considered complete and will not be issued a reimbursement. In signing this section, the applicant is certifying that all information entered on this form, and all submitted supplemental documentation is accurate and complies with the requirements of UCA 19-6-801 through 19-6-823.
