Dear Mr. Beery:

Rockwell Solutions completed well abandonment activities at the above-referenced site in
December 2024 and February 2025.

The following monitoring wells were abandoned:

MW-1 MW-18 MW-31 MW-33
MW-5 MW-19 MW-32 MW-35
MW-7 MW-20 MW-26R MW-36
MW-9 MW-21 MW-28 MW-37
MW-15 MW-27 MW-31
MW-17 MW-28 MW-32

The following remediation wells were abandoned:
MW-2 MW-30 AS-6
MW-3 AS-1 AS-7
MW-6 AS-2 AS-8
MW-23 AS-3 AS-9
MW-26 AS-4 AS-10
MW-29 AS-5

SVE-2 SVE-7

Also, vapor well VW-1 was abandoned.

All wells were plugged/abandoned in accordance with current environmental industry standards
and Utah Division of Water Rights requirements. Each well was abandoned by Rockwell
Solutions personnel. At all monitoring well locations, the well can was removed, the well was
filled with bentonite chips and cut off below grade, and the surface was backfilled and patched to
match the surrounding pavement or material.

Monitoring wells MW-8, MW-24, MW-25, and MW-34 and vapor wells VW-2, VW-3, VW-4,
and VW-5 could not be located and are assumed to have been destroyed.

Figures 1, 2, and 3 show the locations of all of the site monitoring wells, remediation wells, and
vapor wells. All monitoring wells and remediation wells for the site have been properly
abandoned.
