Dear Mr. Ward, Subject: RTCR Level 2 Assessment Day Star Adventist Academy UTAH10012
I would like to thank you for taking the time to meet with me on October 15, 2024, for a Level 2
Assessment (L2) of your water system. Your willingness to accommodate this request is greatly appreciated. Observations:
This site visit was triggered by Multiple Level 1 assessments Triggering Multiple L2 assessments in 2024. Routine and Repeat TC+ EC- samples, Missing Routine, Repeat, & Triggered Source samples. Well source WS001 (Artesian Well) at the time of inspection appears to have its sanitary seal compromised, concreate around well head is missing, and water is flowing up from around
the well casing when pump is not running, when well pump starts the water subsides down into
the area around the well head. Hydropneumatic Tank HD001 (PSI Tank) is large steel 2500-gallon tank and has not been cleaned/disinfected in an extended historical time frame. System is currently on a boil order due to the amount of TC+ bacteriological samples.
Conclusions: 1. TC + samples are likely being caused by the well seal failure. Contamination is entering the distribution system and causing the TC+ samples.
2. Hydropneumatic Tank is likely dirty and possibly containing foreign matter from the
source well problem. Deficiencies: See Deficiency Report, Significant Deficiencies must be corrected within 30 days.

Recommendations:
1. Install Temporary Chlorination Meeting Secondary requirements until source well issue can be mitigated.
2. Follow RTCR and Ground Water Rule with regards to Repeat sampling and Triggered
source sampling following a routine TC+ sample. Contact DDW if not sure on follow up sampling. 3. Follow sanitary sampling protocol of sufficient flushing and disinfection, using a strong
chlorine-based solution (applying, allowing contact time, flushing, sample collection).
4. Inspect and Clean Storage Tank WS 001 on a regular basis, Disinfect as per AWWA C651 standards as an O& M procedure if possible.
