Good Morning/Afternoon,
In the month of DECEMBER, Your water system did not take a routine total coliform sample.
Was a sample taken for this water system? If so, please send those results to ddwreports@utah.gov.
Please note that submitting through CMDP does not always guarantee that the result has been successfully
received. Occasionally, glitches can occur that prevent the result from coming through.
To ensure everything is processed correctly, it would be very helpful if you could send over the sample. This will allow me
to verify that it is in our system and confirm that it is associated with the correct Water System Number, Name, and
Sample Point.
If a sample was taken and you do not have a record of said sample, please contact the lab you worked with to get that
sample and send it into ddwreports@utah.gov.
If applicable: If the Water System was not open during said month, please provide DDW with the month the
System Closed.

Hi Tammie,
A sample was taken for Strawberry Admin Site for December. I have attached the lab report. When the
sample was submitted, our operator put the correct name down, but the wrong water system number. It

looks like it was entered as the Strawberry Bay Rec Complex #26066, when it should have been entered as
#26077. Will you be able to accept this sample for #26077 for the month of December? I reminded our
operator to make sure the correct number gets put down on the lab form.
Also, this system will be closed from January through April. The system will be started back up in May. We
will be sure to submit a seasonal start up form when the system is started back up. This system usually
operates year round, but this year, the Ranger District decided to shut it down for the winter. Is there a form I
need to fill out for that?

Hi Eli,
I see that 26066 has 3 samples taken from December instead of the normal 2 samples. With this email, we will move
forward with changing the system number to 26077.
For 26077 are they only closing for this year January through April or is this going to be a permanent change? They will
also need to complete a seasonal start up form in May 2025.

Thanks for working on that. For 26077, I believe that that closure will only be for this year. I will make sure to
remind that Ranger District that they will need to complete the Seasonal Startup in May.
