Dear Mr. Humphries:
On September 3, 2024, the Division of Waste Management and Radiation Control (Division) received a permit renewal application (DSHW-2024-008103) for the Sunroc Santaquin Landfill. Enclosed
with this letter are our comments and questions pertaining to it. Please submit to the Division a revised permit renewal application with updates addressing our comments and questions.

Section 3.6 Landfill inspection and Monitoring Schedule
This section outlines the schedule for inspections and monitoring, but it doesn’t appear to have examples of the forms. Please provide example forms of, but not limited to, daily incoming
load records; site inspection/monitoring activity (listing what aspects and areas the inspection consists of); and training [including the topic, instructor (or online or video instructor),
list of attendees, and attendees’ signatures, as a minimum].
The only “Training Log” found appeared to be in the SWPPP appendix.
Section 3.2 Waste Handling Procedures
This section states that the scale house operator will weigh each load coming into the landfill and verify that the contents is acceptable for disposal. Please also include in the application
that at least 1% of the loads/day are also recorded as a Random Load Inspection; this should be done daily when the facility is open.
Section 3.11 Record Keeping
This section states that “(c)opies of suggested record keeping forms are attached in Exhibit "E".” Do you mean Exhibit E or Appendix E? Either could not be located.
Inspection and training forms can be in one appendix or provided in the Plan of Operation section.
4.10 Closure and Post Closure Design and Maintenance
This should begin a new section. It should address all closure and post-closure measures. If an appendix is associated with it, it should be referenced and have a cover sheet.
4.17 Financial Assurance Plan
Under this section, a surety bond was addressed, but an Appendix F cover sheet, distinguishing it from other appendices, could not be located. The financial assurance plan can also
be its own separate section in the permit application.
All cover sheets for each appendix appeared to be missing. Cover sheets distinguish itself from other appendices, and a title should be specified on each cover sheet of its content.
