Review, Operator Application, and the Public Notice information for your facility. The second public notice period ended January 17, 2025. Comments were received, but they did not result
in substantive changes to the Permit. The Public Comment Response Summary Document is attached. Therefore, the Permit has been issued as drafted and is effective February 4, 2025,subject
to the right to appeal in accordance with the provisions of Utah Administrative Code, Section R317-9.The Utah Division of Water Quality (DWQ) recommends that Waterleaf Resources, LLC
provide their Sampling Analysis Plan and Quality Assurance Program Plan to the Director as soon as possible so that DWQ can provide feedback. The Demonstration Plant Considerations Document,
