Dear Mr. Finerfrock:
ln response to the September 2,2005letter from Johnathan Cook, I am encloslng a copy
of the site contour map of the Roberts Pond area. The activities covered by the June 10,
2005 As-Built Report for Roberts Pond covered the removal of the old liner from the
pond and installation of the new HDPE liner. The Pond had been in existence since the
original construction of the White Mesa Mill and the earthwork activities creating the
Pond were monitored during the original earthwork construction lrl-1979. The recent
activities did not alter the configuration of the Pond except for minor re-grading
necessary for the HDPE liner installation.

Dear Mr. Frydenlund,
On June 10, 2005 you submitted an As-Built report for Roberts Pond, as required by Permit No
UGW370004, Part I.H.18. This item required:
18. Roberrs Pond As-Built Report - within 90 days of issuance of this Piermit, the Permittee
shall submit for Executive Secretary review and approval a As-Built Report for the
Roberts Pond. Said report shall document:
a) Alt contaminant removal and re-construction activities completed by the Permittee
in 2002,
b) Engineering drawings and specifications to adequately describe and verify the
re c ent re c onstruction effo rt s -
After review of this report, the Executive Secretary may require modification of the
Reclamation Plan in order to provide adequate demolition and decontamination of the
Roberts Pond at the time of facility closure, and to protect public health and the
environment.
In reviewing the submittal, it was noted that engineering drawings were not provided. The only
drawing we received was a schematic drawing of the liner seam locations. We request that as-

built engineering drawings of the pond complete with coordinates, contours, spot elevations, pond
level gauging stations, and construction details be provided. If as-built information was not
maintained for this project, please provide us with a copy of the engineering design drawings, plan
maps, and cross sections.
