Take the next left onto 2400 North. Turn left on North Bulldog road and head south. After approximately 3/4 of a mile Western Rock Products will be on the right side of the road. SOURCE CONTACTS: Chris Rose, Environmental Specialist 385-400-2119 chris.rose@stakerparson.com OPERATING STATUS: The asphalt and concrete plants were operating normally at the time of inspection. PROCESS DESCRIPTION: Asphalt Plant: Sand and gravel are transferred from the aggregate crushing plant to the asphalt batch plant by a front-end loader and dumped into the asphalt plants compartment feeder bins. The sand and gravel are air classified prior to the asphalt binder being added. Asphalt binder is hauled to the site from various refineries in tanker trucks and off-loaded into the hot oil heat exchanger. Lime is trucked in from off-site and pneumatically loaded into the plant's storage silo. Sand and gravel aggregates are batched by weight from the feeder bins onto a conveyor belt and transported to the pugmill. Inside the pugmill, aggregates are mixed with lime. The lime/aggregate mixture is conveyed through a natural-gas fired, ASTEC double barrel counter flow dryer/mixer. If Recycled Asphalt Product (RAP) is being used, it is dumped into the RAP feeder bin and conveyed directly into the dryer, bypassing the pugmill. The dried lime/aggregate mixture and RAP then enters the outside barrel. The lime/aggregate and/or RAP is mixed with asphalt binder to create asphalt. The asphalt exits the rotary dryer/mixer and is conveyed up a bucket elevator to the scalper screen. Any oversize from the scalper screen is fed into a reject bin. The asphalt passing through the screen is fed into hot mix storage silos. The asphalt is gravity fed into haul trucks from the silo and hauled to the construction site. ) $ ' $ ' 2 Recycled Asphalt Product (RAP): RAP is shipped by truck and offloaded in the yard. It is crushed and screened in the recycle crushing circuit. Chunks of asphalt are fed by a front-end loader into a vibrating feeder/grizzly and then conveyed to a crusher. From the crusher the material is conveyed to a triple deck screen, which either feeds material to a jaw crusher, back to the impact crusher, or to a final product storage pile. Material from the storage pile is transported by a front-end loader to the RAP feeder bins for use in the asphalt plant. Material fed to the jaw crusher is conveyed back to the triple deck screen. Aggregate Plant: All permitted aggregate equipment has been removed from this site except for the wash plant. The source is operating a temporary aggregate plant with a 1000-kW generator (see TRP DAQC-710-24). Concrete Batch Plant: Water is put into the delivery mixer trucks. Washed sand and gravel from the aggregate wash plant is loaded into separate bins and drops into the weight hopper at the correct mix. Dry cement from the cement silo drops into the weigh hopper prior to being discharged into the mixer truck. All mixing of the product is done en route to the job site. APPLICABLE REGULATIONS: Approval Order (AO) DAQE-AN102780003-19, dated January 23, 2019 NSPS (Part 60) OOO: Standards of Performance for Nonmetallic Mineral Processing Plants, NSPS (Part 60) I: Standards of Performance for Hot Mix Asphalt Facilities SOURCE EVALUATION: Name of Permittee: Permitted Location: Western Rock Products - Cedar City Facility 851 North Redrock Road 1405 North Bulldog Road St. George, UT 84770 Cedar City, 84720 SIC Code: 1442: (Construction Sand & Gravel) Section I: GENERAL PROVISIONS I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to those rules. [R307-101] I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401] I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions covered by this AO must be reviewed and approved. [R307-401-1] 3 I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by the owner/operator, shall be made available to the Director or Director's representative upon request, and the records shall include the two-year period prior to the date of the request. Unless otherwise specified in this AO or in other applicable state and federal rules, records shall be kept for a minimum of two (2) years. [R307-401-8] I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall, to the extent practicable, maintain and operate any equipment approved under this AO, including associated air pollution control equipment, in a manner consistent with good air pollution control practice for minimizing emissions. Determination of whether acceptable operating and maintenance procedures are being used will be based on information available to the Director which may include, but is not limited to, monitoring results, opacity observations, review of operating and maintenance procedures, and inspection of the source. All maintenance performed on equipment authorized by this AO shall be recorded. [R307-401-4] I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns. [R307-107] I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150] I.8 The owner/operator shall submit documentation of the status of construction or modification to the Director within 18 months from the date of this AO. This AO may become invalid if construction is not commenced within 18 months from the date of this AO or if construction is discontinued for 18 months or more. To ensure proper credit when notifying the Director, send the documentation to the Director, attn.: NSR Section. [R307-401-18] Status: Out of Compliance. Haul road and loader operation area limits have been exceeded. See Section II for more details. There were no modifications to the equipment or processes noted during the inspection. Records are kept and were provided either on site during the inspection or via email after. The source appears to operate the equipment in a manner consistent with good air pollution control practices for minimizing emissions. The source stated that no breakdowns have occurred that would cause additional emissions. An emissions inventory was submitted for 2023, and emissions data are reported below. Compliance assistance was provided to submit documentation for I.8 on August 29, 2024. See the attachment below. Section II: SPECIAL PROVISIONS II.A The approved installations shall consist of the following equipment: II.A.1 Cedar City Facility Aggregate, asphalt, and Concrete Plants II.A.2 Aggregate Plant - new equipment One (1) jaw crusher/feeder, rated at 700 tons per hour (tph). NSPS Subpart OOO equipment Three (3) cone crushers, each rated at 700 tph. NSPS Subpart OOO equipment Five (5) screens, each rated at 700 tph. NSPS Subpart OOO equipment Miscellaneous conveyors. NSPS Subpart OOO equipment One (1) wash plant II.A.3 Hot Mix Asphalt Plant - new equipment One (1) drum mix asphalt plant rated at 500 tons/hour One (1) baghouse rated at 50,000 acfm Six (6) cold feed bins Six (6) 300-ton asphalt storage silos One (1) pugmill with lime silos connected to the baghouse RAP feeder bins Conveyors and scraping screens The above pieces of equipment are subject to NSPS Subpart I requirements Asphalt and fuel storage tanks, and hot oil heater rated at 9 gallons/hour 4 II.A.4 Concrete Plant - new equipment One (1) 12-cubic yard concrete plant with feeder bins, cement and flyash silos with fabric filters One (1) natural gas water heater rated at 7.0 MMBtu/hr One (1) central dust collector Miscellaneous conveyors Status: In Compliance. There was no unapproved equipment observed during the inspection. II.A.2 All permitted aggregate equipment has been removed from this site except for the wash plant. The source is operating an aggregate plant with a 1000 kW generator (see Temporary Relocation/Operation permit DAQC-710-24). II.A.3 There are only 4 cold feed bins. 2 of the asphalt storage silos are for finished product, and the other 4 are for liquid asphalt. II.B Requirements and Limitations II.B.1 The Cedar City Asphalt, Aggregate and Concrete processing facility shall be subject to the following II.B.1.a Unless otherwise specified in this AO, the owner/operator shall not allow visible emissions from any source on site to exceed 20 percent opacity. [R307-201-3] II.B.1.a.1 Unless otherwise specified in this AO, opacity observations of emissions from stationary sources shall be conducted according to 40 CFR 60, Appendix A, Method 9. [R307-201-3] Status: In Compliance. Only minimal fugitive emissions were observed during the inspection. See the attached VEO form. II.B.2 The Aggregate Processing Plant and Associated Equipment on site shall be subject to the following: II.B.2.a The owner/operator shall not produce more than 900,000 tons of aggregate material per rolling 12-month period. [R307-401-8] II.B.2.a.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a new 12-month total by the 20th day of each month using data from the previous 12 months. Records of production shall be kept for all periods when the plant is in operation. Production shall be determined by belt scale records, scale house records, or bucket scale records. The records of production shall be kept on a daily basis. [R307-401-8] II.B.2.a.2 The owner/operator shall weigh and account for all aggregate material prior to the aggregate material leaving the site or being used in another process on site. [R307-401-8] Status: In Compliance. The source stated that all aggregate material is weighed and accounted for prior to the material leaving the site or being used in another process on site. For the rolling 12-month period from June 2023 - May 2024, a total of 381,873.64 tons of aggregate were produced. II.B.2.b Aggregate processing and associated operations shall not occur before 6 AM or after 9 PM each day from November 1 through March 31. [R307-401-8] 5 II.B.2.b.1 Hours of operation shall be determined by supervisor monitoring and maintaining of an operations log. The hours of operation shall be kept on a daily basis and shall include the time the aggregate processing plant starts operation and the time the aggregate processing plant ceases operation. [R307-401-8] Status: Out of Compliance. The aggregate plant did not run at all from January 1, 2024 - March 31, 2024. From November 1, 2023 - December 31, 2023, the source was unable to provide proof that aggregate operations were limited to this timeframe. The source stated that operations never exceeded the permitted 15 hours a day. Since the time of inspection, the source has asked aggregate managers to start logging on/off times. A Compliance Advisory (DAQC-1198-24) was issued on December 9, 2024. The source responded on December 17, 2024. Based on the response, no further action is recommended for this condition. II.B.3 The Concrete Batch Plant shall be subject to the following II.B.3.a The concrete batch plant shall not produce more than 200,000 cubic yards of concrete per rolling 12-month period. [R307-401-8] II.B.3.a.1 To determine compliance with a rolling 12-month total the owner/operator shall calculate a new 12-month total by the 20th day of each month using data from the previous 12 months. Records of production shall be kept for all periods when the plant is in operation. Production shall be determined by scale house records or sales records. The records of production shall be kept on a daily basis. [R307-401-8] Status: In Compliance. For the rolling 12-month period from June 2023 - May 2024, a total of 47,000.81 cubic yards of concrete were produced. II.B.3.b The concrete batch plant and associated operations shall not occur before 6 AM or after 4 PM from November 1 through March 31. [R307-401-8] II.B.3.b.1 Hours of operation shall be determined by supervisor monitoring and maintaining of an operations log. The hours of operation shall be kept on a daily basis and shall include the time the concrete plant starts operation and the time the concrete plant ceases operation. [R307-401-8] Status: Out of Compliance. The source was unable to provide records demonstrating the concrete batch plant operations were limited to this timeframe. The source stated that operations never exceeded ten hours per day. Since the time of inspection, the source has asked concrete managers to start logging on/off times. A Compliance Advisory (DAQC-1198-24) was issued on December 9, 2024. The source responded on December 17, 2024. Based on the response, no further action is recommended for this condition. II.B.3.c The owner/operator shall use a baghouse to control emissions from the weigh hopper loading of the concrete batch plant. Displaced air from the hopper shall pass through the baghouse before being vented to the atmosphere. [R307-401-8] Status: In Compliance. The displaced air from the hopper passes through the baghouse before being vented to the atmosphere. II.B.3.d The owner/operator shall install a fabric filter device on all material storage silos associated with the concrete batch plant. Displaced air from the silos shall pass through the fabric filter devices before being vented to the atmosphere. [R307-401-8] Status: In Compliance. Displaced air from the material storage silos are routed through the fabric filter devices before being vented to the atmosphere. II.B.3.e The owner/operator shall not allow visible emissions from the concrete batch plant baghouse or fabric filters on site to exceed 10 percent opacity. [R307-401-8] Status: In Compliance. No visible emissions were observed from the concrete batch plant baghouse or from any of the fabric filters during the inspection. 6 II.B.3.f The hot water heater shall not exceed 3,000 hours of operation per rolling 12-month period. [R307-401-8] II.B.3.f.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a new 12-month total by the 20th day of each month using data from the previous 12 months. Hours of operation shall be determined by supervisor monitoring and maintaining of an operations log. [R307-401-8] Status: In Compliance. For the rolling 12-month period from June 2023 - May 2024, the hot water heater operated for 718.5 hours. See the attached spreadsheet and email correspondence. II.B.3.g The owner/operator shall not allow visible emissions from the hot water heater on site to exceed 10 percent opacity. [R307-401-8] Status: In Compliance. No visible emissions were noted from the hot water heater during the inspection. II.B.4 All Haul Roads associated with aggregate, concrete and asphalt plant operations on site shall be subject to the following II.B.4.a The haul roads and loader operation areas shall not exceed the following lengths Total haul roads (paved and unpaved) - 1250 ft, and Loader operation areas - 350 ft. [R307-401-8] Status: Out of Compliance. The source stated these length requirements have been exceeded. A Compliance Advisory (DAQC-1198-24) was issued on December 9, 2024. The source responded on December 17, 2024. An Early Settlement Agreement (DAQC-1254-24) was issued on January 7, 2025. The source submitted payment on January 16, 2025. A Signed Early Settlement Agreement (DAQC-068-25) was issued on January 24, 2025. II.B.4.b An operational vacuum sweeper, and/or a water truck shall be made available during each operating day. The owner/operator shall vacuum, or sweep, or flush with water all the paved haul roads on site to maintain opacity limit listed in this AO. The owner/operator may stop flushing the paved haul roads with water when the temperature is below freezing. The owner/operator may stop vacuum sweeping the paved haul roads when the haul roads are covered with snow or ice. [R307-401-8] II.B.4.b.1 Records of vacuuming, sweeping and/or water application shall be kept for all periods when the plant is in operation. The records shall include the following items: A. Date and time vacuuming, sweeping and/or water application were made B. Number of vacuuming, sweeping and/or water application made and quantity of water applied C. Rainfall amount received, if any D. Records of temperature, if the temperature was below freezing. [R307-401-8] Status: In Compliance. Water application is performed as required. Records were reviewed on site at the time of inspection. II.B.4.c All unpaved haul road and loader operation areas shall be covered with road base and shall be water sprayed to control fugitive dust emissions. The treatment shall be of sufficient frequency and quantity to maintain the opacity limit specified in the AO, or unless it is below freezing. Records of water treatment shall be kept for all periods when the plant is in operation. [R307-401-8] 7 II.B.4.c.1 Records of water application shall be kept for all periods when the plant is in operation. The records shall include the following items: A. Date and time treatments were made B. Number of treatments made and quantity of water applied C. Rainfall amount received, if any D. Records of temperature, if the temperature was below freezing. [R307-401-8] Status: In Compliance. Water records were reviewed on site. See the attached example of watering records from June 2024. II.B.5 All Haul Roads and Fugitive Dust Sources associated with aggregate, concrete, and asphalt plant operations on site shall be subject to the following II.B.5.a The owner/operator shall not allow visible emissions from haul roads and fugitive dust sources on site to exceed 20 percent opacity at all times. [R307-205-4] II.B.5.a.1 Visible emission determinations for fugitive dust emissions from haul-road traffic and mobile equipment in operational areas shall use procedures similar to Method 9. The normal requirement for observations to be made at 15-second intervals over a six-minute period, however, shall not apply. Visible emissions shall be measured at the densest point of the plume but at a point not less than 1/2 vehicle length behind the vehicle and not less than 1/2 the height of the vehicle. [R307-205-4] Status: In Compliance. Only minimal fugitive dust emissions were observed at the time of inspection. The source takes steps to minimize fugitive dust by watering roads and restricting the speed of vehicles involved in the operation. See the attached VEO form. II.B.5.b The combined area occupied by all storage piles on site associated with aggregate processing and concrete plants shall not exceed 15.0 acres. [R307-401-8] II.B.5.b.1 The records of the combined area occupied by all storage piles shall be maintained on site and shall be available to the Director or Director's representative upon request. [R307-401-8] Status: In Compliance. Total acreage of storage piles is 10.045 acres. See the attached spreadsheet summary and measurement report. II.B.5.c The combined disturbed areas associated with aggregate and concrete plant operations shall not exceed 7.0 acres. [R307-401-8] II.B.5.c.1 The records of the combined area of all disturbed areas shall be maintained on site and shall be available to the Director or Director's representative upon request. [R307-401-8] Status: In Compliance. Total acreage of disturbed area s is 5.149 acres. See the attached spreadsheet. II.B.5.d The owner/operator shall apply water sprays on all storage piles on site to control fugitive emissions. Sprays shall operate as required to maintain the opacity limit listed in this AO when the temperature is above freezing. [R307-401-8] Status: In Compliance. The storage piles appeared to have been sprayed recently, and there was no fugitive dust coming from the storage piles. 8 II.B.5.e The owner/operator shall comply with all applicable requirements of R307-205 for fugitive emission and fugitive dust sources on site. [R307-205] Status: In Compliance. The source appears to be regularly watering the unpaved haul roads, spraying storage piles, and cleaning track out from the public road. Only minimal opacity was observed during the inspection. See the attached VEO form. II.B.6 All Crushers on site shall be subject to the following: II.B.6.a The owner/operator shall not allow visible emissions from any crusher manufactured prior to April 22, 2008, to exceed 15% opacity, or manufactured on or after April 22, 2008, to exceed 12% opacity. [40 CFR 60 Subpart OOO, R307-401-8] Status: In Compliance. No visible emissions were observed from any crusher at the time of inspection. II.B.6.b The owner/operator shall install water sprays on all crushers on site to control fugitive emissions. Sprays shall operate as required to maintain the opacity limits listed in this AO when the temperature is above freezing. [R307-401-8] Status: In Compliance. Water sprays have been installed as required. II.B.7 All Screens used in the aggregate plants shall be subject to the following II.B.7.a The owner/operator shall not allow visible emissions from any screen manufactured prior to April 22, 2008, to exceed 10 percent opacity, or manufactured on or after April 22, 2008, to exceed 7 percent opacity. [ 40 CFR 60 Subpart OOO, R307-401-8] Status: Not Applicable. All screens associated with the aggregate plant from Condition II.A.2 have been removed from this facility. II.B.7.b The owner/operator shall install water sprays on all screens on site to control fugitive emissions. Sprays shall operate as required to maintain the opacity limits listed in this AO when the temperature is above freezing. [R307-401-8] Status: Not Applicable. All screens associated with the aggregate plant from Condition II.A.2 have been removed from this facility. II.B.8 All Conveyors on site shall be subject to the following: II.B.8.a The owner/operator shall not allow visible emissions from any conveyor (used in the aggregate plants and manufactured prior to April 22, 2008) transfer point on site to exceed 10 percent opacity, or any conveyor (used in the aggregate plants and manufactured on or after April 22, 2008) transfer point on site to exceed 7 percent opacity. [40 CFR 60 Subpart OOO] Status: In Compliance. No visible emissions were observed from any conveyor belt transfer point at the time of inspection. II.B.8.b The owner/operator shall not allow visible emissions from any conveyor (used in the concrete and asphalt plants) transfer point on site to exceed 10 percent opacity. [R307-401-8] Status: In Compliance. No visible emissions were noted from the any conveyor at the Concrete Batch Plant. The asphalt plant was not operating. II.B.8.c The owner/operator shall not allow visible emissions from any conveyor drop point on site to exceed 20 percent opacity. [R307-205-4] Status: In Compliance. No visible emissions were observed from any conveyor drop point at the time of inspection. 9 II.B.8.d The owner/operator shall install water sprays on unenclosed conveyor transfer points on site to control fugitive emissions. Sprays shall operate as required to maintain the opacity limits listed in this AO when the temperature is above freezing. [R307-401-8] Status: In Compliance. The source stated that water sprays are installed on the unenclosed conveyor transfer points. II.B.9 All NSPS Subpart OOO affected Crushers, Screens, and Conveyors on site shall be subject to the following II.B.9.a Within 60 days after achieving the maximum production rate but not later than 180 days after initial startup, the owner/operator shall conduct an initial performance test for all crushers, screens, and conveyor transfer points on site manufactured after August 31, 1983. Performance tests shall meet the limitations specified in Table 3 to Subpart OOO. Records of initial performance tests shall be kept and maintained on site for the life of the equipment. [40 CFR 60 Subpart OOO] II.B.9.a.1 Initial performance tests for fugitive emissions limits shall be conducted according to 40 CFR 60.675(c). The owner or operator may use methods and procedures specified in 40 CFR 60.675(e) as alternatives to the reference methods and procedures specified in 40 CFR 60.675(c). [40 CFR 60 Subpart OOO] Status: In Compliance. Initial performance tests were performed in 2014 on the Fast Pack equipment. See the attachments from DAQC-996-20 for more information. II.B.10 Requirements on Equipment and Activities Associated with the Asphalt Plant II.B.10.a Visible emissions from the following emission points shall not exceed the following values: A. The scalping screen - 10% opacity B. The asphalt plant baghouse stack - 10% opacity C. The lime silo bin vents - 10% opacity D. The hot oil heater - 10% opacity. [R307-401-8] Status: In Compliance. No visible emissions were observed from any of these points at the time of inspection. II.B.11 Asphalt Plant and Baghouse Requirements II.B.11.a The asphalt plant shall control particulate emissions using a baghouse. All exhaust air from the drum mixer shall be routed through the baghouse before being vented to the atmosphere. [R307-401-8] Status: In Compliance. The exhaust air from the drum mixer is routed through the baghouse before being vented to the atmosphere. II.B.11.b The owner/operator shall install a manometer or magnehelic pressure gauge to measure the differential pressure across each baghouse. The static pressure differential across each baghouse shall be between 2.0 to 6.0 inches of water column. [R307-401-8] II.B.11.b.1 The pressure gauge shall be located such that an inspector/operator can safely read the indicator at any time. The pressure gauge shall measure the pressure drop in 1-inch water column increments or less. The pressure gauge shall be calibrated according to the manufacturer's instructions at least once every 12 months. [R307-401-8] 10 II.B.11.b.2 The owner/operator shall record the reading of the pressure gauge at least once per operating day. [R307-401-8] Status: In Compliance. There is a pressure gauge inside the asphalt plant control room. The pressure gauge was at 3.5 inches at the time of inspection. The pressure gauge was last calibrated on May 3, 2024. The asphalt plant operator records the readings daily electronically. See the attached calibration record and examples of daily check. [R307-401-8] II.B.11.c.1 Stack testing to show compliance with the emission limitations stated in the condition above shall be performed as specified below: Emission Point Pollutants Testing Status Test Frequency Asphalt Plant TSP (virgin) * # Baghouse Stack TSP (RAP) * # PM10 (virgin) ** @ PM10 (RAP) ** @ PM2.5 (virgin) ** @ PM2.5 (RAP) ** @ Testing Status * Initial compliance testing is required for the asphalt plant. The initial test date shall be performed as soon as possible and in no case later than 180 days after the start up of a new emission source, an existing source without an AO, or the granting of an AO to an existing emission source that is modified. A compliance test is required on the modified emission point that has an emission rate limit. Compliance testing shall not be required for both virgin and recycled materials during the same testing period. Testing shall be performed for the product being produced during the time of testing. ** Initial compliance testing is not required unless directed otherwise by the Director. # Initial compliance testing is required. Subsequent tests shall only be performed for PM10. @ Test once every five years or sooner if directed by the Director. Tests may be required if the source is suspected to be in violation with other conditions of this AO. Compliance testing shall not be required for both virgin and recycled materials during the same testing period. Testing shall be performed for the product being produced during the time of testing. [R307-401-8] 11 II.B.11.c.2 A. Notification: At least 30 days prior to conducting any emission testing required under any part of UAC, R307, the owner or operator shall notify the Director of the date, time and place of such testing and shall submit a source test protocol to the Director. The source test protocol shall outline the proposed test methodologies, stack to be tested, and procedures to be used. If directed by the Director, the owner/operator shall attend a pretest conference. The pretest conference shall include representation from the owner/operator, the tester, and the Director. B. Reporting: Upon completion of the DAQ accepted testing methods, the owner/operator shall submit a copy of the results from each performance test as conducted to the Director within 60 days after the test has been completed. C. Sample Location: The emission point shall be designed to conform to the requirements of 40 CFR 60, Appendix A, Method 1, or other methods as approved by the Director. An Occupational Safety and Health Administration (OSHA) or Mine Safety and Health Administration (MSHA) approved access shall be provided to the test location. D. Volumetric Flow Rate: 40 CFR 60, Appendix A, Method 2 or other testing methods approved by the Director. E. TSP 40 CFR 60, Appendix A, Method 5 or other EPA-approved testing method, as acceptable to the Director F. PM10 /PM2.5 For stacks in which no liquid drops are present, the following methods shall be used: 40 CFR 51, Appendix M, Methods 201 or 201a, or other EPA-approved testing method, as acceptable by the Director. The back half condensable particulate emissions shall also be tested using 40 CFR 51, Appendix M Method 202, or other EPA-approved testing method, acceptable to the Director. All particulate captured shall be considered PM10 . The portion of the filterable particulate emissions considered PM2.5 shall be based on information in Appendix B of the fifth edition of the EPA document, AP-42, or other data acceptable to the Director. For stacks in which liquid drops are present, methods to eliminate the liquid drops should be explored. If no reasonable method to eliminate the drops exists, then the following methods shall be used: 40 CFR 60, Appendix A, Method 5, 5a, 5d, or 5e as appropriate. The back half condensable particulate emissions shall also be tested using 40 CFR 51, Appendix M Method 202, or other EPA-approved testing method, acceptable to the Director. The portion of the filterable particulate emissions considered PM10 and PM2.5 shall be based on information in Appendix B of the fifth edition of the EPA document, AP-42, or other data acceptable to the Director. The filterable particulate emissions shall be used for compliance demonstration. The condensable particulate emissions shall not be used for compliance demonstration, but shall be used for inventory purposes. 12 G. Calculations To determine mass emission rates (lb/hr, etc.) the pollutant concentration as determined by the appropriate methods above shall be multiplied by the volumetric flow rate and any necessary conversion factors determined by the Director, to give the results in the specified units of the emission limitation. H. Test Conditions All tests shall be conducted in accordance with R307-165-4. [R307-165, R307-401-8] Status: In Compliance. The most recent stack test was conducted on August 15, 2023. The source was running RAP while testing. Results for PM were 4.63753 lbs/hr and 0.02422 gr/dscf. See DAQC-1008-23 for more information. II.B.11.d The asphalt plant shall not produce more than 500,000 tons of asphalt material per rolling 12-month period. [R307-401-8] II.B.11.d.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a new 12-month total by the 20th day of each month using data from the previous 12 months. Records of production shall be kept for all periods when the plant is in operation. Production shall be determined by scale house records or vendor receipts. The records of production shall be kept on a daily basis. [R307-401-8] Status: In Compliance. For the rolling 12-month period from June 2023 - May 2024, a total of 156,187.89 tons of asphalt were produced. II.B.11.e The asphalt plant and associated operations shall not occur before 6 AM or after 9 PM each day from November 1 through March 31. [R307-401-8] II.B.11.e.1 Hours of operation shall be determined by supervisor monitoring and maintaining of an operations log. The hours of operation shall be kept on a daily basis and shall include the time the asphalt plant starts operation and the time the asphalt plant ceases operation. [R307-401-8] Status: In Compliance. The asphalt plant did not run at all from November 16, 2023 - March 31, 2024. See below for HMA run times from November 1, 2023 - November 15, 2023. II.B.11.f The owner/operator shall use natural gas, propane, fuel oil, on-specification used oil as defined in R315-15, or any combination thereof as fuel in the hot mix asphalt plant. [R307-401-8] Status: In Compliance. The source uses only natural gas as fuel in the hot mix asphalt plant. II.B.11.g The sulfur content of any fuel oil burned in the asphalt plant shall not exceed 0.50 percent by weight. [R307-401-8] II.B.11.g.1 The sulfur content shall be determined by ASTM Method D2880-71, D4294-89, or approved equivalent. Certification of fuel oil shall be either by the owner/operator's own testing or by test reports from the fuel oil marketer. [R307-401-8] II.B.11.g.2 The owner/operator shall keep and maintain records of the test certification of sulfur content in fuel oil. Records of the test certifications shall be kept for all periods when the plant is in operation. [R307-401-8] Status: Not Applicable. The source uses only natural gas as fuel in the hot mix asphalt plant. 13 II.B.12 Requirements on the Silo Bin Vents for the Asphalt Plant II.B.12.a All displaced air from the lime silos shall pass through a fabric filter device (bin vent) before being vented to the atmosphere. [R307-401-8] Status: In Compliance. All displaced air from the lime silos passes through the bin vents before being vented to the atmosphere. Section III: APPLICABLE FEDERAL REQUIREMENTS In addition to the requirements of this AO, all applicable provisions of the following federal programs have been found to apply to this installation. This AO in no way releases the owner or operator from any liability for compliance with all other applicable federal, state, and local regulations including UAC R307. NSPS (Part 60) OOO: Standards of Performance for Nonmetallic Mineral Processing Plants Status: In Compliance. Initial performance tests were performed in 2014 on the Fast Pack equipment. See the attachments from DAQC-996-20 for more information. Nozzle sprays for dust suppression are inspected daily, and the dust suppression system is inspected as a whole every month. See the attached inspection from August 2024. NSPS (Part 60) I: Standards of Performance for Hot Mix Asphalt Facilities Status: In Compliance. This applicable federal requirement is satisfied by previous conditions in the AO. See Condition II.B.9.a for more details. The number you’re looking for is in the “actual operating time” column. This is proof that we didn’t run over 15 hours a day between 11/1/23 and 3/31/24. We don’t currently have as slick of a way to pull RMC operation times, but we do log the water heater hours for the plant. This will show the operating hours per day plus a little extra with startup and shutdown time for the heater. We aren’t running all the aggregate equipment on the AO, but I’m having the foreman scan and send over a few days of pre-shift inspection forms for what we are running. After talking with our managers at the site it looks like we haven’t been recording the start stop times of the aggregate and batch plants, we just record total operating hours for the plants. Since the RMC is limited to 6 AM to 4 PM in the winter and aggregate 6 AM to 9 PM, I can provide proof that the RMC never operated over 10 hours a day and aggregate 15 hours a day, just not the actual timestamps. Since your inspection this summer I’ve implemented requirements for the managers to start logging on/off times. For the portable crusher that was recently moved to the site, we haven’t done the initial VEOs yet. We’ll make sure to conduct initial VEOs within the allotted 60 days of startup. After mapping the haul roads and loader operation areas, unfortunately it seems as though we have gone over our length requirements. I will be working to submit an NOI very soon to update the Cedar City AO accordingly. As I’ve mentioned to you before, we strive to remain compliant; I’m happy to try and expedite any way to achieve that. I’ll reach back out with the daily pre-shift inspections soon. The only other portable we’ve had there recently according to my notes was approved in Aug 2022. Lastly, we do keep record of our spray nozzle inspections. Before firing up, the plant foreman completes a pre-shift inspection, one point of which is dust suppression nozzles. Therefore, the spray nozzles are inspected on a daily basis as per our MSHA workplace daily inspections.
