Comment The Permit (DSHW-2019-004201) was reviewed. The facility is permitted to
accept nonhazardous industrial waste generated by U.S. Gypsum
Company. Burning is allowed. Cover is be to applied at least monthly and
recorded as part of the daily operating record.

Comment The 2024 Annual Report was reviewed. 100% of the waste received is
derived from Sevier County. Total Materials Received is noted to be
5,270.00 tons. The current financial assurance is deficient.

General Comments
Comments Though the facility has not been covering waste at least monthly, there has
been a significant amount of improvement. The facility has initiated a cover
log since the last inspection and more waste has been covered. The cover
log demonstrates that waste is being covered monthly for the most part, but
some months have been skipped. The facility has prompted swift action to
cover waste accordingly and keep the cover log updated. The facility did not
display adequate signage, but has since ordered new signage.

The road leading up to the working face has uncovered waste within the base of the
landfill. There has been a significant amount of improvement covering the waste
since the last inspection.
