Dear Jason Wayne Taylor:The Division of Drinking Water (the Division) received the plans and specifications for Price Avenue Water Line from your consultant, Sterling Woodruff and Horrocks
Engineers on December 24, 2024. Written review comments were provided to your consultant on December 26, 2024, January 13, 2025, and January 21, 2025. The Division received the revised
plans and specifications on January 22, 2025. This project has met the conditions for receiving a plan approval.
On December 24, 2024, the Division also received a completed Water/Sewer Separation - Exception Request Form and Water/Sewer Separation - Exception Request Spreadsheetfor an exception
to Rule R309-550-7(1), Separation of Water Mains and Transmission Lines from Sewers and Other Pollution Sources for Utah18032 Distribution System (DS001). This rule states specifically
that the horizontal distance between pressure water mains and sanitary sewer lines shall be at least ten feet and that where a main and a sewer line must cross, the water main shall
be at least 18 inches above the sewer line.
Plan Approval
Our understanding of the project includes the following:
5,800 linear feet of new 12-inch diameter C900 PVC DR18 water line that will connect the Price Avenue Well (WS014) to the existing Utah18032 Distribution System (DS001) in the Division’s
database.
The water line will utilize jack-and-bore steel casing installations for a road crossing and the crossing of the Millcreek Channel.
The water line will not add any ERCs to the existing system.
We have completed our review of the plans and specifications, stamped and signed byMark Edward Chandler, P.E., dated December 23, 2024, and find they basically comply with the applicable
portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for Price Avenue Waterline are hereby approved.
This approval pertains to construction only. An Operating Permit must be obtained from the Director before Price Avenue Waterline may be put in service. A checklist outlining the items
required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of
this project. As the project proceeds, a notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted
to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled.
This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.
Exception to R309-550(7)(1) – Minimum Separation Requirements

Request Spreadsheet.
On the basis of the submitted documents, an exception to R309-550-7(1), Separation of Water Mains and Transmission Lines from Sewers and Other Pollution Sources, is hereby granted for
Utah18032 Distribution System (DS001).This exception may be rescinded any time if new evidence indicates that the conditions of this exception are no longer being met, or a public health
risk exists due to this exception.
Please maintain a copy of this letter with your permanent records for future reference.

Items 1 through 8 below must be submitted to the Division and found to be acceptable prior to operating permit issuance (unless a water line project meets the requirements of R309-500-7
