HomeMy WebLinkAboutDSHW-2024-009304December, XX, 2024 David Villalobos, Registered Agent Oakmont Tire Services, LLC 2240 West 3800 South West Valley City, UT 84119 RE:Request for Information – Quarterly Activity Reports – Oakmont Tire Services Dear Mr. Villalobos: This Request for Information is being sent to your attention as the registered agent of Oakmont Tire Services, LLC (the Respondent). On April 11, 2024, the Division of Waste Management and Radiation Control (Division) issued a Waste Tire Transporter Certificate (DSHW-2024-005165) for the 2024 calendar year. The Respondent has been assigned the Utah Waste Tire Transporter Number TR2024-04. Utah Administrative Code R315-320-4(9) requires that each waste tire transporter submits quarterly activity reports to the Director in an electronic format. To date, the Division has not received any quarterly activity reports for 2024. The purpose of this letter is to obtain more information detailing the quantities and recycler location(s) the Respondent has been transporting to and to determine compliance with the Waste Tire Recycling Act, Title 19 Chapter 6. Within 14 days of the date of this Request for Information, please submit the following requested documentation to the Director:2024 quarterly activity reports including the following information:the number of waste tires collected at each waste tire generator, including the name, address, and telephone number of the waste tire generator;the number of tires listed by the type of tire based one passenger or light truck tires with a rim diameter of 19.5 inches or less, truck tires or tires ranging in size from 7.50x20 to 12R24.5, and other tires such as farm tractor, earth mover, motorcycle, golf cart, and ATV;the number or tons of waste tires shipped to each waste tire recycler or processor for a waste tire recycler;the number of tires shipped as used tires to be resold;the number of waste tires placed in a permitted waste tire storage facility and names of the facilities; andthe number of tires disposed in a permitted landfill, or put to other legal use, and names of the landfills. This is not an agency order and your response to this Request for Information is voluntary. However, if you fail to respond to this Request for Information within the 14-day timeframe, the Director may take appropriate supplemental actions. The matters addressed above, including each letter identified above, all responses, and associated corrective actions, will become part of the compliance record and may be considered in connection with future enforcement actions.
