Jen,Hello! I hope your holiday season was wonderful and the new year is off to a good start.I know these are due today, nothing like getting things done at the last minute.Sorry for the delay.Please see attached information for Bountiful City (UTAH06015) IPS corrections.I have attached the correction notice, IPS report and supportive documents. I am also including an email correspondence for ST009 that should have been corrected in September of 2021.This is also on the correction notice. When I spoke with Kraig today he stated that Zesiger should be listed as inactive. That is not the case in Waterlink for the treatment plant but it is for the well. Do they have to request both be listed as inactive?Let me know if you have any questions or need further information.

Thanks for reaching out about our report. We have been working to make the correcve acons to get the deficiencies.
Correcve acon taken per facility.
ST004 Stoneridge storage. Lid access hatch was replaced and operational with new gaskets.
ST007 Barton Creek storage. Replaced gaskets on both access hatch's.
ST009 Holbrook storage. Gaskets replaced on both access hatch's.
TP001 Zesiger Well. Inactive site. We removed the chlorine cylinders to use at other sites. Nothing to restrain at this point.
WS003 Calder Well #1. Well house & head not secured. We are having a cage built and powder coated that will be installed when it is finished.
WS018 100 East Well. This site is getting the same style cage built as Calder Well. Still have not got a delivery yet but will install when they are rea

Jen,One more item.Disregard the first IPS Corrections sheet.Somehow the two of the pictures for ST004 did not save. This one has two additional pictures of the replacement lid and gasket.
