Box 519 10500 North 14200 West Heber City, Utah 84032 Subject:Plan Approval, Mayflower Booster Station #2 (PF019); Jordanelle SSD, System #26086, File #18719 Dear Max Covey:The Division of Drinking Water (the Division) received the plans and specifications for the Mayflower Booster Station #2 (PF019) from your consultant, Steve Jackson of Jackson Engineering on January 2, 2025. This project has met the conditions for receiving a plan approval. Our understanding of the project is the construction of the Mayflower Booster Station #2 (identified as PF019 in the Division’s database) at the Mayflower Mountain Resort located west of Utah Highway 40 overlooking the Jordanelle Reservoir. This pump station will convey water from the Lower East Park 1.1 MGD Tank (ST005) to the Mayflower Tank #3 (ST021). The pump station will include three (3) vertical turbine pumps each capable of 200 gallons per minute. The pump station will have a total capacity of 400 gpm with a redundant pump. We have completed our review of the plans and specifications, stamped, and signed byNick Graue, P.E., of Aqua Engineering dated April 27, 2022, and find they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for the Mayflower Booster Station #2 (PF019)are hereby approved. This approval pertains to construction only. An Operating Permit must be obtained from the Director before Mayflower Booster Station #2 may be put in service. A checklist outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter. If you have any questions regarding this approval, please contact Dani Zebelean, P.E., of this office, at (385) 278-5110, or me at (385) 515-1464. Sincerely, Michael Newberry, P.E.
