The Division of Waste Management and Radiation Control (Division) has reviewed your renewal application for the Advanced Paving and Construction Class IVb Landfill that was received
by the Division on December 12, 2023 (DSHW-2024-004237). The items detailed in the enclosed document were not addressed in the permit application as required by the Solid Waste Permitting
and Management Rules found in R315-301 through R315-320 of the Utah Administrative Code.
Please provide the requested information or otherwise respond to each of the items listed in the attached document. After responses are received, the Division will continue to review
your permit renewal application.
