When they heard that Lyman Water System was planning on drilling a well, they approached Lyman on the possibility of combining resources. Lyman Water System is open to the idea of combining resources and supplying water to the airport, but didn't have knowledge about the PER (prepared by Cascade) being submitted in their name. So long story short, it is the same well, but I think the previous PER and plan approval should be voided, and a new review on the new (Ensign prepared) PER. The design is a bit different, and I think the smaller 8" well proposed by cascade will not meet the Town's long term needs. Let me know if you have further questions. A statement on page 5 says the saturated aquifer thickness was determined to be 290 feet based on the proposed screen interval. I thought the saturated thickness used in the modeling was 500'. Please fix this reference. Land Use Agreements, Letters of Intent, or Zoning Ordinances The land use agreement signed by Susan Kent does not contain the language required for source protection zone one. It wasn't explicitly stated but if I understand correctly, Lyman Town will eventually own the land within zone one. If Lyman doesn't already own the land shown on Exhibit 1, then the land use agreement signed by the current landowner needs to reflect the language for zone one protections as follows (missing text in bold): I(we), the undersigned landowner(s), acknowledge the Preliminary Evaluation Report/Drinking Water Source Protection Plan for the Lyman Water System Well #1. We agree not to locate or allow the location of any uncontrolled potential contamination sources, as defined in R309-600-6(1)(w) of the Utah Administrative Code (UAC), within zone one. We also agree not to locate or allow the location of any pollution sources, as defined in R309-600-6(1)(v) of the UAC, within zone two unless design standards are implemented to prevent contaminated discharges. The land owner lives in Virginia, so that took a little while to get turned around. Please see attached PER with the following changes from the previous draft. 1. We have not submitted any plans or specifications on this well yet, although we are pretty much done with the design. The project will also involve a new tank, which we are still working on approvals with the environmental process. We could send the well development plans and specifications in for review, prior to the tank, but because of the funding, we are likely to bid the entire project as one. Do you have a preference on if we should send well development plans in now, and then the tank after? (Obviously the equipping request will need to be a separate request after the well development is done). Melissa or Noah, the source hasn't yet been added to SDWIS. Once it has been, you'll need to update the records in WL. Thanks! The Division received the Preliminary Evaluation Report (PER) for the Lyman Well #1 from your consultant, Travis Gadd, with Ensign Engineering, on September 5, 2024. Division staff requested additional information on September 26, 2024 and your consultant provided revisions to the PER on January 20, 2025. We have completed our review of the PER, stamped and signed by Kelly L. Chappell, P.E., and found that it basically complies with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems. The Division concurs with this report. This report must be refined, and a complete Drinking Water Source Protection (DWSP) plan submitted within one year of the date of this letter. Refer to R309-600-13(6) and R309-600-7(1). The Division also received a land use agreement signed by the landowner located within source protection zones one and two, and recorded in the Wayne County Recorder's office on December 19, 2024, which fulfills the source protection requirements necessary for an Operating Permit.
