As required by R307-326-9(7) and 40 CFR 60.592a(e), Chevron Salt Lake Refinery (Chevron) is submitting
the 2'd Quarter 2024 VOC monitoring and leak data for components subject to R307-326-9 (UACR-regulated
components), and lst Half Semiannual 2024 monitoring and leak data for components subject to 40 CFR
Subpart GGGa (NSPS-regulated components).
In accordance with the Consent Decree (C 03-04650 CRB). UACR-regulated valves in light-liquid and vapor
service are inspected quarterly (with the exception of valves in process units that are currently on alternative
skip monitoring in accordance Appendix K of the Consent Decree.
This report provides information required by R307-326-9(7) and 40 CFR 60.487a. Because the previous
UACR quarterly report was already submitted for period ending March 3l .2024, this report contains only 2"d
Quarter 2024 UACR monitoring data. Additionally. this report contains monitoring data for NSPS-regulated
components for the period of January 1,2024 to June 30,2024.

reporting. As per requirements at 60.482-2a the following I I pumps in the tank field unit were
inadvertently marked out of service on 61512023. They were not monitored again until 912612023.
Monitoring is required monthly as per 60.482-2a(a)(1).

reporting. As per requirements at 60.482-7(a)(2), the following 8 valves in the Reformer unit missed
follow up monitoring after the initial monitoring for new components. Initial monitoring was completed
in August. The components were placed out of service on September l5rr'before they were able to be
monitored. Follow up rnonitoring has been completed as appropriate for components brought back in
service. For remaining components, follow up monitoring will be completed once brought back in
service.

l. Citation $60.482-6a, Open-Ended Lines: During the field review of the Isom and Coker on
911112023 and9ll2l2023, seven instances of uncontrolled Open-Ended Lines were found.

Identification and Corrective Actions:
Overlooked components in the VGO and Coker have been identified, tagged in the field, added to
Sky Bridge database. and monitored on 09/1412023.

3. Citation $60.482-7a(a)(2), Missed Initial Monitoring: Newly installed valves missed a second
successive monthly inspection before beginning routine quarterly monitoring.
Identification and Corrective Actions:
Performed successive monitoring for 35XVl43 and 35XVl44 on 1112112023 and 1211212023.
Verified scheduling in Skybridge database is correct for future monitoring event on 35XVl43 and
35XV144.
4. Citation $60.482-7a(c)(2), Post Repair Follow-up Inspection: One ( I ) leaking valve missed the
first and second month post-repair follow-up inspections.
Identification and Corrective Actions:
Performed two months of consecutive monitoring for VOC tag5700 on 111812023 and
1211212023. Changed the leak definition in Skybridge database from 500PPM to 499PPM under
60GGGa rule. This will prevent another missed monitoring post follow-up exercise.
