Subject:Operating Permit, Ductile Iron Pipe replacement, Lower Tank (ST001) Abandonment and PRV station installation. System #07067, File #13541, SRF #3F1879The Division of Drinking
Water (the Division) received a request for an Operating Permit for the subject project from your consultant, Jones and DeMille Engineering on January 17, 2025. The Division issued Plan
Approval for this project on December 14, 2023. The project consisted of replacing approximately 1250 feet of 8-inch diameter Ductile Iron Pipe (DIP) adjacent to Highway 40 with 12-inch
diameter HDPE, abandoning the lower 30K gallon tank (ST001) and installing two Pressure Reducing Valve (PRV) stations to create new pressure zones in the lower part of the system.
The Division has received the following information for the project:
Certification of Rule Conformance with plan approval conditions and changes made during construction were in conformance with rules R309-500 through 550 by a professional engineer.
Contract Record Drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
The Division has determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the DIP replacement, Lower Tank (ST001) Abandonmentand
PRV Station Installation is hereby issued as constituted by this letter. You may now place these facilities in service in your water system.The issuance of this Operating Permit resolves
your water system’s deficiencies related to M020, inadequate system pressure and the associated individual home booster pumps. An updated IPS report isavailable at https://waterlink.utah.gov.
