Box 97 Fairview, Utah 84629 Subject:Operating Permit, Spring Supply Pipeline & Access Hatch Replacement Project (WS001& WS002); Fairview City, System #20012, File #16508 Dear Justin Jackson:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Spring Supply Pipeline & Access Hatch Replacement Project from your consultant, Dave Dillman with Horrocks Engineering on January 16, 2025. The Division issued Plan Approval for this project on September 24, 2024. Our understanding of the project is that approximately 1,200 feet of new 10-inch diameter HDPEpipeline has been installed between the Lower Tollgate Spring (WS002) and the Little Bear Spring(WS003). This new spring supply pipeline removed two aerial creek crossings by keeping thenew pipeline on the south side of Cottonwood Creek. In addition to replacing that section of thespring supply pipeline, this project also replaced the existing access hatches on the Upper Tollgate Spring Box (WS001) and the Lower Tollgate Spring Box (WS002). We have received the following information for the Spring Supply Pipeline & Access Hatch Replacement Project: Certification of Rule Conformance with plan approval conditions by a professional engineer. As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Spring Supply Pipeline and Access Hatch Replacementsis hereby issued as constituted by this letter. You may now place the spring supply pipelinein service in your water system. Please maintain a copy of this letter with your permanent records for future reference.The issuance of this Operating Permit resolves your water system’s deficiency related to Lower Tollgate Spring (WS002) not having an air vent in the spring box.
