Dear Mr. Manning:
On December 9, 2024, the Division of Environmental Response and Remediation (DERR)
received a Subsurface Investigation Report for the above-referenced site. The DERR has reviewed
the information provided and recommends further investigation to determine the full extent
of contamination northwest of the aboveground petroleum storage tanks (APSTs). During the
October 2024 investigation and the July 2024 property assessment, soil and groundwater samples
indicate concentrations above State-established applicable standards. The release has yet to be
defined to the northwest of the APSTs.
The Division Director requires that additional samples be collected to determine the full
extent and degree of petroleum-contaminated soil and groundwater to the northwest of the APSTs.
At least two rounds of quarterly groundwater monitoring of all the project wells should follow the
installation of the new well(s). All environmental samples must be collected by a Utah-certified
groundwater and soil sampler following the sampling protocol outlined in Utah Admin. Code R311-
205. The first report associated with these requirements (full definition of impacts) must be
submitted to our office within 120 days of receiving this letter. Subsequent quarterly groundwater
monitoring reports must be submitted within 30 days of each sampling event. Environmental
samples for gasoline and diesel must be analyzed using the methods indicated in the DERR Table
of Analytical Methods for Sampling.
Additionally, abatement of the impacted soils at the APST fill-port area should occur. Your
Utah-certified consultant should discuss a limited excavation plan with the DERR project manager
and determine if a Corrective Action Plan (CAP) is warranted for this work. If no CAP is required,

this soil abatement and confirmation sampling should occur in conjunction with the additional
investigation requirements above. The reporting of the fill-port soil abatement should be included
in the first report outlined above.
