The EWA is issued by the Executive Director of the Utah Department of Environmental Quality (DEQ). The application will be processed by the Division of Environmental Response and Remediation (DERR). Complete all sections and submit the application to the DERR. Please submit the application well in advance of acquiring, developing or occupying the property. The Applicant must meet all the requirements of the Act and supporting regulations.
