Subject:Operating Permit, 2024 Culinary Water Improvement Project;
Ephraim City, System #20011, File #13644
Dear Jeff Jensen:The Division of Drinking Water (the Division) received a request for an Operating Permit for the 2024 Culinary Water Improvement Project from your consultant, Layne
Reed with Franson Civil Engineers on January 8, 2025. The Division issued Plan Approval for this project on March 18, 2024. Our understanding of the project is that approximately 4,200
feet of 12-inch diameter HDPEtransmission pipeline has been installed from the North Well (WS017) along 400 West to the southand connected to the existing distribution system at approximately
300 North. Approximately 4,500feet of new 8-inch diameter C900 PVC and 2,600 feet of new 10-inch diameter C900 PVC pipinghas been installed in the same described area to provide “looped”
distribution lines for the system.
We have received the following information for the Distribution Improvement Project:
Certification of Rule Conformance with plan approval conditions by a professional engineer.
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the 2024 Culinary Water Improvement Projectis hereby issued
as constituted by this letter. You may now place the new water lines in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
