INCIDENT SUMMARY
On 12/11/24 the environmental group was alerted to a diesel spill which was investigated. I piece of heavy equipment had recently refueled and moved
into a sloped area. Investigators thought it was an isolated incident due to sloshing of diesel from the equipment's breather. As a precautionary measure
they closed underground lines associated with AST(s) proximal to the refueling station. The crew excavated the contaminated soil. On 12/18/24 about
15:45 maintenance was called out due to more suspect diesel observed in a inside a sump. Crew closed the lines and completed repair to the sump
yesterday and reopened the lines. Further trench investigation down to a depth of 18 inches, has raised suspicion that the leak may be originating with
the underground lines associated with AST(s), and they have again closed the lines. Though the source has not yet been located, they have again closed
the nearby AST lines. The facility has deployed booms and pads in/near the ditch/retention pond. The pond discharges to the facility oil/water separator
which discharges to a facility outfall. There has been no apparent release to the facility outfall at this time. Personnel have collected several water
samples and are awaiting results. They are investigating the lines at this time. An integrity test was performed on 12/6/24.
