Jasin,
I took a look at our file on MTX. It looks like we determined in 2014 that the release was not from a regulated petroleum
storage tank and did an internal DEQ Transfer of the site to DSHW for oversite. Beyond the 2014 transfer, we do not have any
records on the cleanup. We call it a closure for the release, but that just means it is no longer regulated by DERR. The site did
have an older release HPR in 1992, that was cleaned up under DERR oversight and received a No Further Action (NFA)
Closure letter in 1996. I have included a few of the documents below. I hope this helps.

Attached is a sketch of the soil remediation field at Georgia-Pacific Coiporarion's Sigurd, Utah gypsum
wallboard/plaster manu^turing facility. As shown, the area is approximately 175' x 125". The
approximately 1.050 yd' of contaminated soil was spread unifoimly over the area.
Pursuant to our telephone conversation today, Georgia-Pacific hereby requests approval of a sampling and
analysis plan Co fulfill die requirement ofyour June 20,1996 letter to Mr. Sbaun Williams. As we
discussed during our telephone ccmversation, Georgia-Pacific proposes to divide the remediation field into
12 equal area« and draw a single composite sample from the areas. The sample wjU be analyzed for Total
Petroleum Hydrocarbons and BTEXN-
Please let me know as soon as possible if this course of action is acceptable.

Comments Melissa Turchi spoke with Mi cheat Eiian/Tctra Tech regarding release.
Site is currently a Gypsum Sheet Rflgk Plant. Mike called in a relea.se
because they found some contaroination on site. He look some samples
and is awailinit the final lab results. He believes that the- contamination
could have crime from the hydraulic lift system onsite. The sump is
different in that it is an open sump full of hydraulic oil. There is a
possibilly that the contamination could be from an old LUST release at
the_sj.ie or maybe even from ASTs. He is also workinii with Pat
Sheehan/DSHW on Ihc release, since he didn't know if the hydraulic
system is a regulated tank. Once he rccicvcd the analytical data he
would submit the results to hoih ihe DERR and SHW.

280.10(b)(3) specifically says "hydraulic lift tanks" are EXCLUDED from Part 280 regulations (as an example of "equipment

I came down to talk to someone regarding a potential UST/LUST site in Sigurd, UT, and you were the person I was referred to!
Sorry about that...but I wanted to give you a heads up that I was on the phone with Georgia-Pacific today regarding
contamination at the site. I told them that I would try to talk to someone in your shop about it, so, if you don't mind, maybe we
can talk about it later this afternoon (I have a meeting from 2:30 til 3:30 pm or 4 pm) today, and if that doesn't work, then
maybe tomorrow?
I will call back Georgia-Pacific's consultant and let them know that I should have more information tomorrow. I hope that works
for you. Let me know if you are not the person I should talk to also.

SUBJECT: Transfer of all hard copy LUST information to the UST Notification case file
Georgia-Pacific Corporation, located at South State Street, Sigurd. Utah
Facility Identification No. 2000058, Release Site MTX
I have reviewed the file for the above-referenced facility, which includes information provided by
the facility owner or others. My recommendation that no further action is required at the facility
is based on the review of the information contained in the case file. A suspected release was
reported to the Division of Environmental Response and Remediation. However, further
investigation (outlined below) has indicated a new release from a regulated underground storage
tank (UST) has not occurred. I recommend no Reporting and Remediation compliance schedule
be sent to the responsible party, a Leaking Underground Storage Tank (LUST) file be maintained
in the database LUST module file, and all hardcopy information be filed in the LUST case file. In
addition, no closcout letter will be sent to the responsible party since the site has not been tracked
as an active LUST site.
Case file information justifying no further action includes the following: a release was reported
based on visible soil impacts during excavation near an open hydraulic lift oil sump. Hydraulic
lifts are excluded from the federal UST regulations and any release from a hydraulic lift is not
regulated as a UST release. The Division of Solid and Hazardous Waste has been managing this
project since the release was reported.
