Everett Director ERRL-0091-24 March 14, 2024 Charles Philips 1054 South 450 West Price, Utah 84501 Re: Approval for Work Plan and Budget No. NPQ-06 Petroleum Storage Tank Fund Wellington Single Stop Shell, located at 690 East Main Street, Wellington, Utah Facility Identification No. 5000005, Release Site NPQ Dear Mr. Philips: The Division of Environmental Response and Remediation (DERR) received the Work Plan and Budget (WP&B) and Work Plan Approval Application and Agreement on February 21, 2024. The WP&B dated February 8, 2024 was prepared by Underwood Environmental Consulting, Inc. (UEC). This WP&B is for the proper abandonment of the four remaining groundwater monitoring wells at the above-referenced site. The DERR approves the WP&B up to $3,687.09. A copy of UEC’s budget spreadsheet is attached. Please submit a short letter report documenting groundwater monitoring well abandonment to the DERR by May 20, 2024. Requests for Petroleum Storage Tank (PST) reimbursement must be submitted in accordance with Utah PST Rule R311-207-4 and R311-207-5. Please reference WP&B No. NPQ-06 on your request for Reimbursement. Payment for work conducted under this WP&B will be for actual time and materials expended up to, but not exceeding, the amount of the approved line item budget. Payment shall be limited to documented costs that are customary, reasonable, legitimate, and directly related to the work tasks performed. Requests for payment must be received by the DERR within one year of the date that the work was performed or reimbursement shall be denied in accordance with Utah PST Rule R311-207-3.
