Dear Mr. Stauffer:

The Division of Waste Management and Radiation Control completed a 30-day public comment period
on the Site Management Plan (SMP) for the property located East of Sheep Lane and North of SR-112
in Tooele, Utah (the Property) on January 28, 2021. No comments were received during the 30-day
public comment period. The SMP for the Property is hereby approved for implementation.

Enclosed is the signed original Environmental Covenant (EC) for the Property with my signature. As
outlined in Paragraph 17, the Covenant must now be signed by representatives of RG Lakeview, LLC
and recorded with the Tooele County Recorder’s office.

Please ensure a date stamped copy of the recorded EC is submitted to the Division within thirty days of
recordation.
