HomeMy WebLinkAboutDDW-2025-000085 January 6, 2025 Justin Blackett Nephi City 21 East 100 North Nephi, Utah 84648 Subject:Plan Approval, Well Equipping, Worwood Well REPL-1 (WS012); Nephi City, System #12003, File #13432 Dear Justin Blackett:The Division of Drinking Water (the Division) received the plans and specifications for the Worwood Replacement Well REPL-1 (WS012) from your consultant, Parker Vercimak with Jones & DeMille Engineering, on December 19, 2024. The Division issued Plan Approval for drilling Worwood Well REPL-1 on January 3, 2024. Written review comments on the well equipping plans were sent on December 23, 2024, and revised plans and specifications were received on January 2, 2025. Our understanding of the project is that the original Worwood Well (WS011) was producing excessive sand, so the City decided to abandon that well and drill a replacement well within 150 feet of the original well. The replacement well has been drilled to a depth of 600 feet with a 10-inch diameter steel production casing and intermittent stainless steel wire-rapped screen.. A grout surface seal was placed from ground surface down 110 feet and #8 SRI Supreme gravel pack was placed in the annular space from 110 feet to the bottom of the well. The well was test pumped for 24 hours at 250 gallons per minute (gpm) with 190 feet of drawdown. The well will be equipped with a 40 hp submersible pump capable of producing 200 gpm at 460 feet of total dynamic head. This replacement well will utilize the existing well discharge piping and well house that served the original well. This replacement well is identified as Worwood Well REPL-1 (WS012) in the Division’s database. We have received the following information for Worwood Well REPL-1 (WS012): Certification of well seal. Well driller’s report (well log). Well driller’s report for abandoning the original well. Aquifer drawdown test results (step drawdown test and constant-rate test). New Source Chemical Analysis of the well water. Plans and specifications for equipping the well. Submersible well pump; 40 hp motor, 200 gpm production Pitless adapter design and specification. Modifications to existing well house We have completed our review of the plans and specifications, stamped, and signed by Parker Cade Vercimak, P. E., and dated August 21, 2024,and find they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for equipping Worwood Well REPL-1are hereby approved. This approval pertains to construction only. An Operating Permit must be obtained from the Director before Worwood Well REPL-1 may be put into service. A checklist outlining the well approval process, including the items required for issuing an operating permit for this well is enclosed for your information. Approvals or permits from the local authority or county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter. New Source Chemical Analysis The initial new source chemistry data of the well water showed a detection for toluene. As a result, upon the issuance of an operating permit, the Worwood Well REPL-1will initially be required to be sampled for volatile organic compounds (VOCs) each quarter that it provides water to the distribution system.
