IS&R/CARR FORK SITE
1.0 INTRODUCTION
This 2023 Annual Inspection and Maintenance Report has been prepared by Atlantic
Richfield Company (Atlantic Richfield) in accordance with the Long-Term Operation and
Maintenance (LTOM) Plan (ARC, January 2022) incorporated by reference into the
Consent Decree (Civil Action No. 2:16CV982DBP) (CD, 2016) for the International
Smelting and Refining Company (IS&R) site (Site). Per Section 2.2.5 of the LTOM Plan,
this Report documents inspection and maintenance (I&M) activities conducted in 2023.
The Site comprises three areas: the Pine Canyon Conservation Area (PCCA); the Lincoln
Residential Area, and portions of the Tooele Valley Railroad (TVRR) outside the PCCA
boundary (see Figure 1, Figure 1.2).
The 3,000-acre PCCA of open space land encompasses 1,200 acres of reclaimed and
remediated land associated with the former smelter operations Site. Significant Site
reclamation and remediation activities were completed in 1986, with additional remedial
and ongoing maintenance actions completed since. CERCLA response actions were
implemented in 2006-2007 (EPA, September 2007), which included, among other things,
placement of a clean soil cap over impacted areas, reshaping of the surface for storm
runoff control, installation of drainage control structures, and revegetation of disturbed
areas (see Figure 2).
A LTOM Plan was prepared in 2010 (updated in January 2022) as the controlling
document for implementation of operation and maintenance activities that ensure the long-
term effectiveness of the Site’s remedial action. Per Section 2.0 of the LTOM Plan, regular
Site inspections and maintenance activities are conducted to ensure that remedial
features remain in place and are performing as intended, an institutional controls (ICs)
evaluation is performed to confirm to the established ICs continue to remain effective in
protecting human health and the environment, and annual groundwater monitoring is
conducted.

Objectives outlined in Section 1.1 of the LTOM include the following:
• Maintain site conditions that are protective of human health and the environment.
• Conduct operation, inspection, and maintenance of the engineered controls.
• Conduct maintenance, inspection, and enforcement of the land restrictions and
other Institutional Controls (ICs) necessary for protecting remedies.
• Conduct long-term groundwater monitoring to demonstrate the effectiveness of
the remedies put into place and confirm the smelter site is not a source of ground-
water contamination in the future.
• Meet regulatory requirements for the post-remedial action conditions.
This Annual Report addresses the LTOM Plan’s requirements to annually report on the
Site I&M activities and ICs evaluation for 2023. Groundwater monitoring is addressed in a
separate annual report, which for 2023 was submitted to EPA on January 25, 2023. This
report is organized as follows:

2.0 SUMMARY
I&M activities conducted during 2023 included inspections of areas with remedial features
or to otherwise monitor the effectiveness of the Site remedy, cover repair, Smelter Road
gate repair, fence repair, additional fencing at the slag-pile, surficial soil sampling of the
TVRR grade within the PCCA, and an evaluation of the established ICs to confirm they
continue to be effective in protecting human health and the environment.
3.0 SITE INSPECTIONS
Per Section 2.2 of the LTOM Plan, Site inspections are required to be conducted in areas
where “waste is left in place”, including areas within the PCCA, the TVRR Trestle, and the
TVRR Zions Farm Area at least annually to confirm that remedial action features remain in
place and are performing as intended. The 2023 Annual Inspection was conducted in the
second quarter of 2023 (See Inspection Forms in Appendix A, Inspection Site
Photographs in Appendix B).

3.1 Pine Canyon Conservation Area (PCCA)
In accordance with Section 2.2.2 of the LTOM Plan, the 2023 Annual Inspection
and other periodic inspections were completed at the PCCA to examine the
condition of the Site remedial cover/cap materials, fences, gates, information signs,
runoff control berms and channels, drainage structures, fill slopes, vegetation,
erosion potential, evidence of unauthorized use, and to otherwise confirm the
remedy continues to function as intended. The inspections focused on looking for
anomalies that could jeopardize the integrity of the remedial actions such as soil
creep, bulging, erosion, differential settlement, or rock degradation.
The 2023 Annual Inspection was conducted in June 2023. LTOM Plan Inspection
forms were completed during the annual inspection (See Appendix A). The
Annual Inspection details observations throughout the entire Site and the PCCA
and photographs were taken to document site conditions (see Appendix B) at
specific locations as specified by the LTOM Plan (see Figure 2.1). Site
maintenance is scheduled and performed as needed, based on observations
during inspections and notification from stakeholders of issues warranting further
attention.
Inspections also looked for physical evidence of uses that are inconsistent with the
allowed uses as defined in the 1994 Conservation Easement (ARC, April 1994, as
most recently amended in 2011). Per the Conservation Easement, the Utah
Division of Wildlife Resources (UDWR) is responsible for maintaining PCAA areas
and improvements, including repair of damaged fencing, gates, and signage. In
accordance with the LTOM Plan and the Conservation Easement, Atlantic Richfield
notifies and coordinates with UDWR on needed fence repairs, e.g., by providing
UDWR with photos and coordination on fence break locations as they are identified
(See Appendix C).
The PCCA boundary fence requires occasional maintenance due to breaks caused
naturally, by livestock, and wildlife or in locations cut by individuals trying to gain
access at locations other than designated entry points. In 2023, Atlantic Richfield
elected to perform fence repairs along the north, east, and portions of the south
boundaries. (See Figure 4.1).
Three signboards exist at different locations around the boundary of the PCCA to
provide information about the area to the public. Two of the signboards are located
along Anaconda Road, and one signboard is in place at the parking lot near the
south end of Blue Peak Road.
Vegetation was inspected over the PCCA including remediated cover areas (See
Inspection Forms in Appendix A). The inspections generally showed robust
vegetative cover across the PCAA and on remediated areas of the Site. However,
certain small areas were identified where cover soil appeared discolored and/or

had sparse/barren vegetation (See discussion in Section 4.0). The UDWR
continues to spray for noxious weeds each year and both mows fire breaks and
sprays vegetation to limit emergence of vegetation along the PCAA perimeter near
current subdivisions.
Riprap and vegetation-lined channels are all functioning properly, without
noticeable breaching or erosion. Diversion berms are intact and functioning
properly. Culverts under roadways are functioning properly on the site.
UDWR mows a fire break around the western perimeter of the site each year to
minimize potential fire(s) from spreading onto or off of the PCCA.
3.2 Tooele Valley Railroad
In accordance with Section 2.2.2 of the LTOM Plan, Atlantic Richfield inspected
two areas along the TVRR grade outside the PCCA where waste was left in place
(TVRR Trestle Area and TVRR Zions Farm Area) to confirm that remedial action
features remain in place and are performing as intended. This included field
inspection of cap materials, drainage structures, fill slopes, and vegetation. These
TVRR areas are in good condition with no observations of anomalies that could
jeopardize the integrity of the remedial actions, nor identification of any
maintenance actions needed (See Inspection Forms in Appendix A).
4.0 MAINTENANCE ACTIVITIES
Maintenance activities implemented during 2023 are described below with additional
details shown on Figure 4.1,4.2,5,7 and photographs in Appendix D. Maintenance
activities implemented during 2023 are the result of certain anomalies noted in
inspections, which were performed to maintain the integrity of the remedial actions in
accordance with the LTOM Plan requirements.
4.1 Smelter Road Gate
In 2023, the security gate approximately 1000 feet northeast of the Tooele Gun
Club entrance (see photographs in Appendix D) was again severely damaged due
to an isolated incident of vehicular vandalism, but was promptly repaired/replaced.
Access through this gate at the end of the Tooele County public right-of-way is
provided to Kennecott, UDWR, and public utilities, but the gate prohibits access to
general public vehicle traffic along the road through the PCCA to the Kennecott
property.
Prior to the security gate, there was a longstanding problem of the public
transporting furniture/appliances, household garbage, and other trash or
construction debris into the area south of the slag pile and illegally dumping these
items into ravines along the road. As a result of the security gate, no illegal
dumping of trash or debris in this area was noted in 2023.

4.2 Fence Maintenance
In 2023, Atlantic Richfield elected to perform additional maintenance/repair of
fencing along the northern, eastern, and portions of the southern fence where the
fence was damaged or had loose strands (See Figure 4.1).
In May-June 2023, Atlantic Richfield installed an additional 8-foot chain-link fence
and signage along the more accessible access points of the slag pile area to
further deter public access to the slag pile (See Figure 4.2 and photographs in
Appendix D).
4.3 UDWR 2023 Maintenance Activities
The UDWR sprayed for noxious weeds over the site. UDWR also mowed a
firebreak near public roads and residential subdivision areas, primarily along the
western boundary of the PCCA.
In May 2023, UDWR completed installation of a boundary chain link fence along
the north boundary of the Carr Fork Subdivision (see Figure 4.1).
4.4 Soil Sampling
During the 2023 Inspections, Atlantic Richfield noted areas where surface soil
along the TVRR grade within the PCCA appeared discolored and/or had
sparse/barren vegetation.
Following a discussion and EPA’s concurrence in September 2023, Atlantic
Richfield performed XRF soil sampling in October 2023 along the portion of the
TVRR within the PCCA for metals (lead and arsenic). A soil sample was collected
approximately every 100 ft along the centerline of the rail grade and at certain
additional locations based on observations in the field. Soil samples were analyzed
using an XRF and compared to site action levels (See Figure 5.1-5.4). For quality
assurance, 10% of the field samples were submitted to Pace Analytical Laboratory
for verification analysis (See Appendix F). Maintenance activity will be performed
in 2024 for locations that exceeded site action levels, consistent with the Site
LTOM Plan and previous remedial actions.
4.5 Atlantic Richfield 2023 Maintenance Activities
Maintenance activities implemented in 2023 were based on recommendations
included in the 2022 I&M Report (ARC, March 2023) . In November-December
2023, Atlantic Richfield completed cover soil repair at various locations as shown
on Figure 7.1-7.8, and photographs shown in Appendix D. These maintenance
activities primarily addressed certain areas exhibiting erosion, discolored surface
material, or sparse/barren vegetation, and that exceeded site action levels based
on sampling performed in 2022. Maintenance included spreading/tilling lime,
placement of 18-in soil cover, mulching, and reseeding of new cover and disturbed
areas. Maintenance work was performed in accordance with the LTOM.

5.0 EVALUATION OF INSTITUTIONAL CONTROLS (ICs)
ICs are non-engineered instruments such as administrative and legal controls created to
minimize the potential for human exposure to contaminants and/or protect the integrity of
the remedial actions. Over time following remedial action construction, the role of ICs
becomes ever more important in helping to maintain the long-term protectiveness of the
Site’s remedy. In accordance with Section 2.2.4 of the LTOM Plan, Atlantic Richfield
conducted the 2023 ICs inspection for the PCCA, TVRR, and Lincoln Residential Area to
evaluate if the established ICs continue to remain in effective in protecting human health
and the environment. The following sections summarize the 2023 ICs evaluation.
5.1 Pine Canyon Conservation Area Institutional Controls
The Conservation Easement established by Atlantic Richfield and UDWR was
implemented to protect remedial features, restrict the type of use allowed on the
property, and preserve, protect, and enhance native wildlife and the natural,
scenic, open space, and educational values of the property. An environmental
covenant (PCCA, June 2010) was also added to the property in 2010.
The Conservation Easement prohibits, among other things, subdividing, building,
drilling, mining, filling, excavating, removal of topsoil and other materials, dumping,
and commercial, industrial, and agricultural use and all other uses inconsistent with
the stated purpose and objectives of the Easement. The 2010 covenant prohibits
motorized vehicles, bicycles, livestock grazing, swimming or wading, fires,
dumping or leaving garbage in the PCCA. New groundwater wells, except for
environmental groundwater monitoring, are also prohibited by both ICs. The ICs
specifically allow authorized vehicle use for maintaining remedial features,
monitoring, sampling, and other activities directly associated with Site I&M.
In conjunction, these ICs effectively restrict public use of the PCCA to non-invasive
hiking, horseback riding, wildlife observation, and hunting. UDWR also uses the
PCCA as an educational area for hunter safety training (e.g., staging Chukar hunts
during the year). These ICs continue to remain effective in protecting human health
and the environment while allowing certain public use opportunities.
5.2 Tooele Valley Railroad Institutional Controls
Environmental covenants have been recorded on each of the affected properties
where remedial actions took place along the TVRR corridor. In 2009, an
environmental covenant (TVRR, June 2009) was placed on approximately 1.87
acres of the larger TVRR grade area to provide long-term controls over the former
Trestle Area, and in 2010 an environmental covenant (TVRR, May 2010) was also
added to an area of the Zions Farm property where approximately 3,000 feet of
waste was capped in place along the TVRR grade. Both covenants prohibit actions
that would disturb the capped areas.

Portions of the TVRR corridor are owned by Tooele City and other portions are
owned by private entities. Monitoring of the TVRR ICs includes inspecting the site
for any damage to remedial improvements and working with Tooele County,
Tooele City, and private property owners to educate them on restricted activities.
To date, the ICs are working as intended and remain effective in protecting the
capped areas as no intrusive activity was found during the 2023 annual inspection
or in previous inspections.
In accordance with Section 2.2.4 of the LTOM Plan, and in conjunction with EPA
and UDEQ, Atlantic Richfield continues to notify private property owners to ensure
they are aware of the ICs on their property and limitations on land use.
5.3 Lincoln Residential Area Institutional Controls
ICs in the Lincoln Residential Area (i.e., Pine Canyon Township) consist of the
Developer Guidelines/Pine Canyon Environmental Overlay Zone and a Controlled
Groundwater Area (CGA) (See Section 5.4). The Tooele County Planning and
Zoning Department (TCPZ), in consultation with the Tooele County Health
Department (TCHD), administers the Developer Guidelines for properties within
the Pine Canyon Environmental Overlay Zone. The Developer Guidelines are
codified in Chapter 29 of the Tooele County Code. For properties within the
Overlay Zone, a review of the environmental condition and possible remedial
action is required as part of the development or building construction approval
process. Applicants anticipating development within the Lincoln Residential Area
must first meet with and provide proposed development plans to the TCPZ to
determine if the property to be developed or otherwise improved is subject to the
requirements of the Overlay Zone. If TCPZ determines the development or other
improvements are within the Overlay Zone, TCPZ will refer the applicant to the
TCHD, which will review previous sampling data and other information on
environmental conditions associated with the subject property, and then determine
if remedial action is warranted. The TCHD also maintains a website with
educational information on lead exposure and prevention
(https://tooelehealth.org/preventing-lead-poisioning/).
In addition, the TCHD monitors groundwater development in the Lincoln
Residential Area, restricting private well installation in areas that may have
elevated naturally occurring arsenic concentrations (as described further in Section
5.4 below).
The TCPZ and TCHD continue to effectively administer the Developer
Guidelines/Pine Canyon Environmental Overlay Zone ICs. Atlantic Richfield has
verified that Ordinance Chapter 29 of the County Code is still in effect and serving
its intended purpose. The Developer Guidelines are available at the Tooele County
Planners office and posted on the TCHD website. The Developer Guidelines are
also readily accessible to the public on common search engines. The TCHD and
TCPZ continue to educate and clarify the intent of the ICs for the public. Atlantic

Richfield also independently monitors the area for new development in the Overlay
Zone to confirm that the Guidelines are being followed and contacts stakeholder
agencies as necessary if any concerns or issues arise. Atlantic Richfield continues
to respond to TCHD’s requests to clarify site-specific information when developers
contact them during their project planning.
5.4 Groundwater Institutional Controls
The LTOM Plan (Appendix G) outlines a CGA with three distinct groundwater
zones, each with varying levels of restrictions. The CGA was created to prevent
extraction of groundwater with naturally elevated arsenic concentrations in the
area. Within CGA Zone 1, installation of wells other than for monitoring or other
environmental purposes is prohibited by the conservation easement and
environmental covenant for the PCCA.
Appendix G to the LTO&M Plan states that wells drawing water strictly for
environmental monitoring purposes may be drilled within CGA Zone 2, but private
and public (municipal) wells installed for the purpose of providing culinary water will
not be permitted within CGA Zone 2. TCHD, Regulation #5: Individual Water,
prohibits Individual Water Systems, and State regulations (UAC R655-4-9.2.1 and
UAC R655-4-1.1) are intended to prohibit municipal wells in Zone 2.
Within CGA Zone 3, a.k.a. the Advisory Zone, Appendix G to the LTO&M Plan
states that entities drilling a well are to be made aware that some wells in the
vicinity have had measurable concentrations of arsenic, and Individual Water
Systems (a.k.a. private wells) must be permitted and inspected according to TCHD
Regulation #5: Individual Water Systems. When a new well is drilled within Zone 3
the TCHD permits the use of the well after a water quality sample is collected and
subsequent chemical analysis shows the water meets drinking water standards,
including arsenic. The water quality sample data is subsequently provided by the
TCHD to EPA and UDEQ.
It appears that the CGA zones continue to be working as intended by the LTOM
Plan. No known new wells were installed in Zone 1 or Zone 2 during 2023. Figure
6 indicates that the Zone 2 boundary remains a valid representation of areas with
naturally elevated arsenic concentrations above the MCL where all groundwater
wells should continue to be prohibited or restricted. The TCHD provided no
groundwater data in Zone 3 for 2023. However, all data previously provided by
TCHD to date indicate arsenic concentrations are below the arsenic MCL within
the Zone 3 wells.

6.0 CONCLUSION / RECOMMENDATIONS
Overall, the IS&R Site remains in good condition, as constructed remedial features and
ICs that have been put in place continue to be effective and serve their intended purpose.
This section summarizes the 2023 I&M conclusions and provides recommendations for
certain maintenance activities in 2024 based on the identified findings. Additional
Site/Agency correspondence relevant to the 2023 I&M activities is included in Appendix
E.
6.1 PCCA
Conclusion
Overall, the PCCA remedial action features remain in place and are functioning as
intended. Soil cover maintenance was completed in 2023 to address sampled bare
areas with exceedances of arsenic and/or lead action levels. This maintenance
work will continue to preserve the integrity of the remedy. The evaluation of ICs for
the PCCA indicates that the Conservation Easement and the environment
covenant continue to remain effective in protecting remedial construction,
protecting human health and the environment, and in promoting the long-term
objective of creating an attractive open space for wildlife. As part of their oversite
work at the PCCA, in 2023, UDWR sprayed for noxious weeds, repaired boundary
fence, and mowed fire breaks near public roads and residential subdivision areas,
generally along the western boundary of the PCCA.
Recommendations for 2024 Maintenance Activities
• UDWR and Atlantic Richfield will continue to monitor for any vehicular and
cycling trespass and other unauthorized site access, and UDWR will
enforce the PCCA use restrictions via informational signage and reporting
to local law enforcement, as necessary.
• UDWR (as previously discussed with EPA, UDEQ and Atlantic Richfield)
will continue to mow a wide fire break and implement other fire break
measures around the Carr Fork and Conservo Subdivisions early in the
season before grasses dry out, typically prior to June 15th, 2024.
• Atlantic Richfield will continue coordination with the UDWR to
repair/reconstruct damaged site fencing.
• In 2024 Atlantic Richfield will address areas along the TVRR grade within
the PCCA where 2023 XRF sampling results were above Site action levels.
Consistent with the LTOM Plan and previous remedial and maintenance
actions, these areas will be addressed via placement of cover soil,
repair/installation of stormwater run on and drainage controls to reduce
erosion, and revegetation of the cover soil and borrow areas. This

maintenance work will involve the development of borrow areas within the
PCCA, near the TVRR grade, to facilitate efficient implementation of the
work and to minimize truck traffic and disturbance of other vegetated areas
within the Site to the extent practicable. These maintenance activities will
be reported in the 2024 I&M Report.
6.2 Tooele Valley Railroad
Conclusion
The TVRR Trestle Area and the TVRR Zions Farm Area are in good condition and
have not been disturbed. Both environmental covenants remain in place on the
affected parcels.
Recommendation
• In conjunction with EPA and UDEQ, in 2024 Atlantic Richfield will notify the
current owners of these TVRR parcels to ensure that the owners are aware
of the environmental covenants on their property and the land use
limitations associated therewith (See Appendix E).
6.3 Lincoln Residential Area
Conclusion
The Developer Guidelines appear to be functioning and implemented as intended.
Atlantic Richfield responds to TCHD requests as needed to clarify site-specific
information as developers contact them during project planning.
Recommendations
• Per Section 2.1.4 of the LTOM Plan, TCHD, TCZD should continue to
monitor development in the Lincoln Residential Area, facilitate proper
implementation of Developer Guidelines and educate property owners on
their application when agricultural land is changed to residential use, and
perform other duties as outlined in the LTOM Plan.

6.4 Controlled Groundwater Area
Conclusion
The IS&R/Carr Fork 2023 Annual Groundwater Sampling Report (ARC, January
2024) provides conclusions and additional details about the 2023 groundwater
monitoring event.
It appears that the CGA zones continue to be working as intended by the LTOM
Plan. No new wells were installed in Zone 1 or Zone 2 during 2023. Figure 6
indicates that the Zone 2 boundary remains a valid representation of areas with
naturally elevated arsenic concentrations above the MCL where all groundwater
wells should continue to be prohibited or restricted. All data provided by TCHD to
date indicate arsenic concentrations are below the arsenic MCL within the Zone 3
wells
Recommendations
• As noted in prior annual reports, a potential gap in CGA coverage may exist
where a public water supply well could be proposed, permitted, and drilled
in Zone 2. Atlantic Richfield therefore believes that a formal CGA or similar
regulatory designation should be set up by the Utah Division of Water
Rights and/or Toole County to be consistent with CGA Zone 2 restrictions.

Description Details: Conservation Area
Trash/Debris From the road check for garbage and other debris at the bottom of the slag pile and note any large items (cars, boats, etc.) that may have been stolen and abandoned. Evaluate and
address concerns.
Box Culvert Check the box culvert debris or restricted flow issues. Check for structural deficiencies. Evaluate and address concerns as needed.
Safety Berm Check the integrity of the 5' Safety Berm along edge of the slag pile. Evaluate and address concerns as needed.
Information Signs Check restricted entry and information signs for presence and condition. Evaluate and address concerns as needed.
Regraded/
Backfilled Areas
Check regraded areas for signs of erosion, settlement and degradation of cap material. (Backfill against the former smelter foundations, for purposes of the annual and five-year review, is
considered a regraded area.) Evaluate and address concerns as needed.
Borrow Areas Check borrow areas for vegetation cover (density and specie variety) and erosion. Evaluate and address concerns as needed.
Backfilled Erosion Rills Check backfilled erosion rills for damage; ensure constructed diversion berms are keeping the rill from re-occurring. Evaluate and address concerns as needed.
Cap Check capped areas for sustainability (vegetative cover, etc.), bio-intrusion or human caused damage. Evaluate and address findings as needed.
Channels Check channels and channel revetment for erosion caused damage, damage by other natural causes or by human related activity. Evaluate and address concerns as needed.
Diversion Berms Check constructed diversion berms to ensure adequacy; check berm for overtopping, erosion or breach. Evaluate and address concerns as needed.
Fence Check all Conservation Area fencing including fencing along roads, gun club, etc. for damage or deficiencies.
Vegetation Check work areas of stressed vegetation. If encountered check the area for evidence of waste material. If waste is observed take appropriate action.
Erosion Check work area with special consideration of reclaimed areas for evidence of erosion that has caused or may cause exposure to waste materials.

Re: Pine Canyon Conservation Area Boundary Fence Inspection

During the 2023 Annual Inspection of the Carr Fork/Pine Canyon Conservation Area (PCCA)
multiple locations were noted where the fence line was in disrepair. These locations have been
identified with GPS coordinates, as shown in the enclosed table. Additionally, a map depicting the
locations (see Figure 1) and an attached Photolog showing the fence line locations requiring
maintenance are enclosed.
New chainlink fence installed by UDWR along the north boundary of the Carr Fork Subdivision
is also shown on Figure 2. Based on previous discussion and cooperation, Atlantic Richfield
Company is willing to work with UDWR on additional chainlink fence in certain key areas such
as those shown on Photos 22 – 28 of the attached Photolog.
Additionally as previously discussed in the field, Atlantic Richfield installed additional chain-link
fence at certain locations at the slag pile (Figure 2).

All sample aliquots were received at the correct temperature, in the proper containers, with the appropriate preservatives, and within method specified holding times, unless qualified or notated withinthe report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental sampleshave been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control are within established criteria except where addressed in this case narrative, a non-conformance form or properly qualified within the sample results. The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.

Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor.
Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges.
Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG.
Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the resultreported. Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte.
Uncertainty (Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material.
Sample Chain of Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section foreach sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates andtimes of preparation and/or analysis.
