HAND DELIVERED
At approximately l5:40 on December 3,2024, Tesoro Refining and Marketing Company LLC in Salt
Lake City, Utah (Tesoro) experienced excess opacity at the SRU incinerator stack. These emissions
occurred while the Tail Gas Treatment Unit (TGTU) experienced an unplanned shutdown when the
J-245A fan shut down due to a high vibration alarm.
An initial notification of the breakdown was made by Chris Kaiser to Jeremy Marsigli at 08:44 hours
on December 4,2024.
CAUSEN{ATURE OF RELEASE AND CORRECTIVE ACTIONS:
Operators implemented procedures to safely secure the unit. The redundant l-245B was started and
procedures were implemented to restart the TGTU.
SUMMARY OF DEVIATIONS:
SRU Incinerator Stack Opacity. Incident Date: December 3,2024. Incident Time: At l6: l5 a Method 9 reading indicated 6-minute average
opacity of 42%o. Operational Limit: l0% opacity as a six-minute average
