RE: Exception Report Clean Harbors Aragonite, LLC EPA Number – UTD 981 552 177 Dear Mr. Hansen,

In accordance with Module 1 condition 1.A.3 of Clean Harbors Aragonite’s (CHA) State RCRA Permit, 40 CFR 262.42, 40 CFR 761.217 and R315-262-42, this correspondence serves as written notification of two (2) shipment of hazardous waste or PCB regulated waste to other facilities that CHA has not received a signed copy of the received manifest from the destination
facility. The manifest information and efforts CHA are taking to resolve each issue can be seen
below:
• Manifest 019849711FLE – CHA has tracked this manifest, and it is still on route to the
destination facility. The last correspondence on this manifest was on November 15, 2024,
and the last known location of the manifest is in Kimball, NE.
• Manifest 019849714FLE – CHA has tracked this manifest, and it is still on route to the
destination facility. The last correspondence on this manifest was on November 14, 2024,
and the last known location of the manifest is in Spring Grove, OH. A copy of the manifest has been attached.
I certify under penalty of law that this document and all attachments were prepared under my direction or supervision according to a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate,
and complete. I am aware that there are significant penalties for submitting false information, including the possibility of a fine and imprisonment for knowing violations.
