of an unmanifested waste container received for disposal at Clean Harbors Aragonite, LLC (CHA). 1. EPA Identification Number, name, and address of the facility:
• UTD981552117
• Clean Harbors Aragonite, LLC
• 11600 N. Aptus Road, Aragonite, UT 84022 2. Date facility received waste: 10/6/2024
3. The EPA Identification Number, name address of the generator and transporter (if available)
• Generator: NA
• Transporter #1 Grant County Public Works WA 4. A description and the quantity of each unmanifested hazardous waste the facility received:
• 4 X CF’s – light bulb boxes: containing light bulbs 5. The method of treatment, storage, or disposal, for each hazardous waste: Pending additional investigation, the waste will remain in storage and will be disposed of by incineration. 6. Explanation of why the waste was unmanifested: The facility unloaded this container on October 2, 2024. It had no barcode, or any indication of who the generator was. CHA attempted to resolve through communication with transporters and CHA was unable to do so in the 15-day period. CHA opened the container and discovered that inside the boxes were a mix of light bulbs
and light lamps. CHA will continue to investigate who the generator of this waste is and if it is unable to be determined, CHA will profile, create a barcode, and destroy this container.
