The estimated cost to complete the proposed scope of work is provided in Table 1. Groundwater monitoring well locations are shown in Figure 1. We have included a Petroleum Storage Tank Trust Fund Work Plan Approval Application. Upon your review and approval of this work plan, please sign the attached application and return it to Wasatch Environmental. ENVIRONMENTAL SCIENCE AND ENGINEERING PROPOSED WORK PLAN AND BUDGET FOR MONITORING WELL REPAIR AMERICAN FOOD MART SALT LAKE CITY, UTAH 1. INTRODUCTION This Work Plan presents a brief description of the tasks required to complete monitoring well repair additivities at monitoring well MW-9 at the site. Wasatch was informed by the Salt Lake City Public Works that this well had been damaged. The pictures of the damage were provided to the PST Fund. This work plan includes a cost estimate (Table 1) that has been prepared for the following tasks: • Task 1 – Project Management, Work Plan, and PST Claims • Task 2 – Monitoring Well Repair Activities • Task 3 – Report Preparation 2. SCOPE OF SERVICES The scope of services detailed below provides information regarding tasks required to complete the monitoring well repair activities. All activities described herein would be completed under the direction of a Utah Certified Underground Storage Tank (UST) Consultant. 2.1 Task 1 - Project Management, Work Plan, and PST Claims Preparation of this work plan and budget, Petroleum Storage Tank (PST) claims, obtaining the required Salt Lake City permits, project coordination, and project management are included in the first task associated with the proposed work at the American Food Mart release site. 2.2 Task 2 – Monitoring Well Repair Activities Wasatch will remove the surface completion consisting of concrete and a damaged steel well vault at the MW-9 location. The demolition debris will be disposed of at a local landfill. A new 12-inch, traffic- rated, flush-mount, steel well vault will be placed in new concrete to replace the removed surface completion. Given that MW-9 is located in the roadway of 1700 South, this work will require that we obtain a Salt Lake City Traffic Control permit and a Public Way permit. Additionally, repairs will require the closure of the eastbound (southern) lane of 1700 South. 2.3 Task 3 – Report Preparation Wasatch would prepare a report detailing field activities and procedures, appropriate figures, summarizing results of the monitoring well repair activities. 3. DELIVERABLES The anticipated deliverables identified in this proposed Work Plan include the following: • Payment vouchers with backup documentation. • One Monitoring Well Report presenting the results of the repair activities. PERSONNEL The key Wasatch personnel for this proposed Work Plan are as follows: • Utah Certified UST Consultant and Senior Geologist: Blake Downey, P.G. Christopher Nolan, P.G., or Michael Cronin, P.G.; • Utah Certified UST Groundwater and Soil Sampler Staff Geologist: Jake Scott, Anna Fiorini, Rachel Kubina, Ryan Christiansen, or Jeff Hessburg. 5. SUBCONTRACTORS The subcontractors for the proposed work are as follows: • Utah Barricade (traffic control plan and required barriers set up, take down, and rental). 6. EXCEPTIONS TO WORK PLAN, ANTICIPATED PROBLEMS, OR SPECIAL REQUIREMENTS Wasatch makes no exceptions to this proposed Work Plan. In the event significant resistance or problems arise during the completion of the work proposed herein, Wasatch would communicate with the DERR to evaluate alternative courses of action. 7. CONFLICT OF INTEREST Based on Wasatch’s present knowledge of the site location and conditions, no personal or corporate conflict of interest has been identified for the performance of the scope of services described in this proposed Work Plan. Wasatch would contact the DERR immediately if any potential conflicts of interest are identified during the performance of this proposed Work Plan. 8. COST ESTIMATE The estimated cost by Task for completion of the work described in this proposed Work Plan is presented in Table 1. The proposed budget would require adjustment if the scope of services identified herein is modified or the assumptions prove incorrect. Wasatch would not perform activities outside the scope of services described in this proposed Work Plan without prior approval from the DERR.
